Chronic disease conditions such as diabetes, hypertension, and chronic obstructive respiratory disease remain the significant morbidity burden and medical cost determinant in the US (Holman, 2020). Despite the evolution of the field of medicine, there are still the systemic, behavioral, and educational problems that are the impediments on the successful disease control in the patient. With Family Nurse Practitioners (FNPs) specifically, one would have to handle these barriers to improve the results and encourage evidence-based and equitable healthcare. FNPs play a crucial role of determining and averting the effects of socioeconomic disparity, deficiency of quality health care, and illiteracy. By removing these obstacles, FNPs helps patients to be empowered to be in charge of their own treatment and increases compliance. Philosophical Influences The analytic tradition is my philosophical orientation, which pays attention to logic, empirical research, and evidence-based thinking. It is relevant to the scientific nature of advanced nursing practice and encourages the systematization of patient outcomes and interventions. Analytic philosophy allows the FNP to manage the obstacles to chronic disease management using quantifiable information, including adherence levels, quality indicators, and indicators of care coordination (Rutty, 1998). Nonetheless, understanding of the lived experiences that are not purely quantifiable are also needed in the nursing practice. Although the analytic part is concerned with rationality and data, with the introduction of the continental thought, the empathy and human understanding are increased. Nursing philosophy, according to Duff (2011), presents the fundamental framework by which theoretical and clinical knowledge are combined into practice. To the FNP, the combination of analytic thinking and intuitive understanding permits evidence-based, yet humane care, in response to objective information as well as the subjective experience of patients who are chronically ill. Ethical Framework Ethical theory, which informs this praxis, is deontological ethics. Deontology, which is built on the ground of duty morality, places more weight on professional duties and ethical consistency instead of focusing on particular results (Ahmad and Islam, 2024). Such a framework fits the practice of the FNPs in the best possible way; in this context, the clinician is often faced with decisions to be taken inside a system with limited options, and in the meantime, adhering to the highest ethical standards. Deontology considers the responsibility to honor patient dignity, offer fair access to care, and advocate vulnerable population in the management of chronic disease despite institutional constraints.
