Vision Statement The strategic plan for this new healthcare service aims to further the following organizational vision: To establish the healthiest community in the state where all people, with or without housing or insurance, have fair access to integrated, person-centered care and have a chance to reach the best health and well-being. 2. Mission Statement The mission of ________________________ is to: The Community Health Outreach & Integration Program (CHOIP) mission statement is to decrease health disparities and enhance health outcomes of the under-/uninsured homeless population by offering accessible, trauma-informed, and comprehensive healthcare services that are effectively integrated with community-based housing and social support networks. 3. Identify three potential strategic goals/directions to guide development of your strategic plan. Proposed strategic goal/direction 1: Improve Access and Care Coordination through creation of low-barrier access points and a centralized case management system to move clients up and down the continuum of care. Proposed strategic goal/direction 2: Better Health Results in Chronic Conditions through implementation of specific, research-based mental health, substance use disorders, and chronic physical illness interventions common in this population. Proposed strategic goal/direction 3: Community and System Integration through the development of formal relations with housing authorities, shelters, and social service agencies to implement the social determinants of health. 4. Program Description Provide a summary of the planned new healthcare program and a description of the business processes it supports. The CHOIP is a mobile and clinic-based integrated care initiative that focuses on the homeless population in particular. It is a program that aids in supporting the fundamental business processes, such as patient intake/registration, clinical service delivery, referral management, data collection to report on, and communication with cross-sector partners, redesigned through the low-barrier trauma-informed prism. 5. Customer Satisfaction 5a. Identify the end-users of the investment in this new healthcare program. The core end-users will be the uninsured homeless people of our community. Secondary end-users are clinical staff caring for this population and staff of partner agencies (shelters, soup kitchens) who deal with this population. 5b. Briefly describe the process used to assess end-user/customer satisfaction (i.e., decreased ED usage, improved primary care access, satisfaction surveys, etc.) The satisfaction will be measured with the help of short, orally-administered questionnaires on the basis of a Likert scale, and at the point of care, the criteria will be dignity, respect, and accessibility. Quantitative indicators will encompass the decline in ED use in the context of ambulatory care-sensitive cases and greater numbers of individuals taking part in primary care. 6. Strategic Goals 6a.
