NU 668 Roles and Issues for Advanced Practice SWOT Analysis Student Name: Strengths Enhanced provider handoff communication is likely to improve the chances of continuity of care to the patients through the continuum of care. The cross-functional perception of the needs and symptoms of the patient will enable a higher level of coordination, which will save time and meet the highest standards of the quality of care and patient safety. The healthcare facilities are becoming more willing to develop a culture that respects and works to achieve patient safety. When viewed in this light, better handoff communication between providers is a timely idea. Beyond the positive effects on the part of the patient, it would enable the organizations involved in this improvement to give a better fit with the current regulatory requirements and ethical standards as a means of reputational benefits. Considering the environmental fit, as discussed above, another key advantage of adopting the proposed change is to ensure alignment with growing trends of the interprofessional nature of patient care. The adoption of IPEC standards to streamline the provider handoff will foster a culture of shared accountability, clarity of role, and mutual trust. Ultimately, the complementarity across teams from different departments tends to improve. Weaknesses One of the weaknesses that is quite critical is the fragmented implementation of electronic health systems. The issue is that not every psychiatric facility (where I served) has embraced the concept of EHR platforms throughout the facility, including both outpatient and inpatient facilities, which complicates the process of ensuring that the necessary patient information is available on time. Mental status change, medication change, or behavioral risk factor that is not updated and saved in the providers’ systems will increase the likelihood of omissions in the handoff process. The other major challenge is the fact that there are still no uniform protocols across units or departments. Whereas some teams may use structured tools, other teams may be highly reliant on habit, memory, or informal verbal communication, which endangers the quality and completeness of handoff information, leading to highly inconsistent information. Without the coordination of the systems, there will be inconsistency in the practices of handoff, and this results in a lack of information or misinterpretation of information. One of the frequent issues is the time pressure and the overwhelming workload. The clinicians usually carry out handoffs when they are changing their shifts or when there is a peak workflow, causing them to rush when communicating....
