Mental status change, medication change, or behavioral risk factor that is not updated and saved in the providers’ systems will increase the likelihood of omissions in the handoff process. The other major challenge is the fact that there are still no uniform protocols across units or departments. Whereas some teams may use structured tools, other teams may be highly reliant on habit, memory, or informal verbal communication, which endangers the quality and completeness of handoff information, leading to highly inconsistent information. Without the coordination of the systems, there will be inconsistency in the practices of handoff, and this results in a lack of information or misinterpretation of information. One of the frequent issues is the time pressure and the overwhelming workload.
