Social Determinants of Health (SDOH) Affecting Johnson’s Family Health Status
           The SDOH are environmental conditions that influence an individual’s health, functioning, and quality-of-life outcomes (Figueroa et al., 2020). Examples are income and social protection, education, access to housing and basic amenities, food insecurity, work-life conditions, racial segregation, and gender inequality (Figueroa et al., 2020). The various SDOH affecting Johnson’s family includes social, economic, and personal factors. Financial challenges prevent the family’s access to quality health. Despite being a middle-class family, sometimes, they undergo financial problems because most of the finances are used to educate their children. Lack of enough money has prevented Mrs. Johnson from seeking proper medication to manage diabetes. This problem has also reduced the number of health checkups. Mr. Johnson also has weight issues but does not consider it a significant health problem. This ignorance is inappropriate because being overweight can lead to other health issues like high blood pressure, heart disease, diabetes, and kidney problems. 
           Additionally, Johnson’s family lacks adequate social support to influence them to make appropriate health decisions. Although the family has access to a balanced diet, they all like buying foods from restaurants. Foods from restaurants are often linked with unhealthy food choices, attributed to obesity and weight gain (Johnson, 2018). Proper family support can help change where the family buys food. Regarding employment, Mr. and Mrs. Johnson have to work hard to support the family. They seem to work long hours, which affects the rest of both parents. Inadequate rest can result in lasting physical and mental health problems.
           The impact of SDOH on Johnson’s family is that they have made it hard for the family members to go for regular medical checkups. Instead, the family is limited to one medical evaluation per year. This limitation presents a health risk to the parents who need frequent follow-up. Mr. Johnson also risks further damage to his health by not taking his body weight seriously. The children are also likely to experience stress because of their parents’ conditions. These factors are prevalent for this family due to lack of adequate money, ignorance, and the African-American culture of hard work. 
Age-Appropriate Screenings for each Family Member
           Screeners may conduct screening by evaluating the five core social needs related to health. It involves asking the family valid screening questions associated with the identified social needs, lifestyle choices, food, transportation, safety, and utilities. It is worth noting that the screeners often adopt screening tools to undertake the screening process. Since every family member is an adult, it will be imperative to ask them open-ended questions.
