Ms. Fisher, an 82 years old woman has had medical conditions in the past. First, she has a history of chronic congestive heart failure (CHF). She has been hospitalized four times in the past six months due to the aggravation of her CHF. Secondly, she has a history of atrial fibrillation. Third, she has a history of hypotension. Fourth, she also suffered dyspnea and had been discharged from hospital the previous Saturday after a 3-day stay to treat this condition. As a result of her dyspnea, she has been instructed to use an oxygen concentrator to give her enough oxygen.
Ms. Fisher at the moment has four conditions that need the intervention of a nurse to help her manage them. First of all, her body retains a lot of water making her body to be swollen and also an increase in weight. A condition referred to as “Oedema”. As a result her Lasix dose has been increased. She needs to lose some weight and do some physical exercises to manage her condition. Hypotension: People who have a reading of 90/60 or less are considered to be hypotensive. Ms. Fisher’s blood pressure reads 90/56. With the help of a nurse, she needs to monitor her body’s response to medication to control her blood pressure. This is because of the side effects of high dosage and a patient’s motivation to take anti-hypertensive drugs. (White, Duncan, & Baumle (2011)). Atrial fibrillation: to reduce the occurrence of this condition, Ms. Fisher would need to maintain a good habit to rest physically and emotionally. This is to reduce the demands of oxygen by her body which makes her hear beat fast and irregularly. (Kowey et al (2010)). Dyspnea: since this problem is associated with her breathing system, she needs to be trained proper methods to increase intake of oxygen in the body. The patient needs to learn appropriate ways of breathing and coughing to enhance clearance of secretions. She also needs to learn other methods like yawning to increase air intake to the body. These methods will greatly reduce the occurrence of going short of breath. Also the patient will learn to avoid activities that demand hard breathing. (Mahler & Donn...
