Introduction
The PPACA of 2010 has several provisions that seek to bind together the provision of quality care with the cost of care. It directs its attention, more resources, and incentives towards promoting elements of care that also comprise the backbone of healthcare provision and practice (American Nurses Association, 2010). These include patient-centered or holistic care (including family and community), continuity of care, integration and coordination across all settings of healthcare provision, chronic disease management, education of patient, information management, and prevention and wellness care. There are three emerging and innovative models for delivering care that are contained in the PPACA that is Accountable Care Organizations (ACOs), Medical or Health Home, and Nurse Managed Health Centre.
Accountable Care Organizations (ACOs)
These are local healthcare providing organizations that bear accountability for 100% of a defined patient expenditures and care. ACOs may comprise of specialists, physicians providing primary care, specialists, and the hospitals that collaborate in providing care that is based on evidence in a coordinated approach.
