The United States is regarded as performing worse when compared to other industrialized states. The differences arise due to many factors. Based on the Commonwealth Fund findings, the U.S. scores lowest in terms of several domains which includes equity, administrative efficiency, access, health care outcomes, and care process (Schneider et al. 2017). Even though the U.S. spends a sizable percentage of its GDP on healthcare, it has a wide array of issues in regards to the quality of care.
5 Domains
The five domains are crucial indicators to reveal how a country has performed in regards to different aspects of its healthcare policy. The report by the Commonwealth Fund compares the U.S. with other countries such as Canada, Germany, Norway, Sweden, United Kingdom, New Zealand, Switzerland, Netherlands, Germany, and Australia using seventy-two indicators which are part of the five domains (Schneider et al. 2017). Netherlands, Australia, and the U.K. comprise the top three.
Health Care Outcomes
This is a measure that defined as the changes in health which are from specific health interventions. Based on the report, the U.S. ranks last in comparison to other industrialized states. The U.S. ranks worse on specific aspects like life expectancy at age 60 and infant mortality (Schneider et al. 2017). Additionally, it has also undergone the least improvements in this variable during the past decade. Despite this, the U.S. is ranked better in regards to mortality rates for 30-day in-hospital care after a stroke (Schneider et al. 2017). It also ranks better in regards to the breast cancer survival rates within five years.
Care process
The care process entails the steps that caregivers follow when caring for a patient (World Health Organization, 2020). Sweden ranks last while the U.K. ranks first in regards to care process. The U.S. is at the middle, and some of its strengths, when compared to other counties, is collective decision-making between specialist and primary care providers, a good doctor-patient relationship, end of life-discussions, preventive measures, and chronic disease management (Schneider et al. 2017). Despite this, it ranks worse when it comes to information flow between social services, specialists and primary care providers. The U.S. also has high hospital admission rates which can be avoidable.
Access
Access refers to the timely use of health services to attain the desired results. The U.S. ranks last, and this is attributes to low measures in regards to various indicators like affordability and timelessness (Schneider et al. 2017). The other countries are doing better when it comes to affordability, which has resulted in higher rates of access to healthcare.
Administrative Efficiency
Administrative efficiency involves the capacity of the care organization to achieve the required results with minimal use of resources such as funding and personnel. Many patients in the U.S. are not happy with the country’s administrative efficiency, and a majority of U.S. doctors have issues due to coverage restrictions (Schneider et al. 2017). Other countries such as Norway and the United Kingdom were ranked higher, while France being the only country where people reported issues with administrative efficiency.
Equity
Equity entails providing care without quality differences due to the patient’s attributes such as ethnicity, socioeconomic status, gender, or geographic location. The healthcare system should reduce inequalities by improving the distribution and level of health (Chan, 2009). In regards to this measure, the U.S. and Canada have larger variances between higher-income and lower-income adults in regards to the measures patient-centered care, financial issues to care, and timeliness (Schneider et al. 2017).
