Wolraich et al. (2019) stated that the first-line treatment for children ages six and above should be a combination of medications, including methylphenidate, the primary drug prescribed to the patient, and parent training in behavior management (PTBM). However, the patient experienced worsening symptoms. Hence, the following action is to titrate the dose daily until the third day up to a week to ensure that the patient will receive the maximum dose of methylphenidate. Methylphenidate with the brand name Methylin or Ritalin has an initial dose of 2.5 to 5mg/ day that can be titrated to 0.1mg/kg/dose or 10-50mg/kg/week to a maximum of 60mg per day in patients who weigh less than 50kg or 100mg per day in patients who weigh more than 50kg. This should be given in two to three doses every day for a week then the patient should return for a follow-up (Brown et al., 2018).
However, suppose the symptoms continue to worsen after titrating methylphenidate. In that case, the patient shall be given amphetamine, with the brand names Adderall or Evekeo, 5mg once or twice a day. This can be titrated by 2.5mg per week with a maximum dose of 40mg daily (Brown et al., 2018).
