Emotionally Focused Therapy for the Patel Family
Emotionally Focused Therapy (EFT) is an evidence-based, structured couples therapy focusing on emotional bonds and attachment needs. Developed by Sue Johnson and Leslie Greenberg, EFT helps couples identify negative interaction cycles, develop emotional responsiveness, and create a secure emotional bond. Dr. Sid Patel and Priya Patel from the Patel family presented for therapy due to work-related stress, emotional disconnection, and challenges in managing professional and personal roles. Their interactional pattern is a pursuer-withdrawer cycle whereby Priya seeks emotional intimacy, and Sid withdraws due to stress and perceived criticism. This paper explores their presenting issue, conceptualizes their distress from an attachment-based framework, and offers a treatment plan using EFT interventions. The therapy session demonstrates the application of structured therapeutic interventions to facilitate emotional reunion and conflict resolution. Lastly, this paper reflects on the counseling process, highlighting the therapist's strengths and areas of improvement and evaluating EFT effectiveness with the Patel family.
First Section: Family Description and Presenting Problem
Background of the Patel Family
Dr. Sid Patel and Priya Patel are married in their forties and have three children, ages 6, 13, and 15. Sid is a surgeon, and Priya is the manager of their jointly owned medical clinic, which demands a lot of their time and energy. Their demanding professional lives have caused them to grow emotionally apart, and they struggle to balance work and family life. The stress of keeping up with their careers, household chores, and parenting has strained their relationship, and both partners feel unheard and unsupported. Gradually, their communication became transactional and about logistical issues instead of emotional connection. Priya is starting to feel underappreciated as a professional and caretaker, and Sid feels exhausted and pressured to deliver at high levels at work and home. Emotional stress has created frequent arguments, and they are moving further away.
What Brought the Couple to Therapy?
The Patel family sought therapy due to ongoing relational distress stemming from emotional neglect, frustration, and lack of appreciation in their marriage. What required constant communication for failure was their inability to convey their emotional needs, which caused misunderstandings and resentment. Sid drew in Priya and rejected her when he was more focused on work than family, signaling that she was unimportant to him. Sid also thought he was protecting the family, trying his best to make them feel secure, and his sacrifices and efforts would be appreciated without complaining. The session also touched on an argument that went too far for no apparent reason: unmet emotional needs. Sid, who had an emergency surgery, could not leave work, and Priya had asked him to come back home early for their son's school event. Priya's frustrations and disappointments became fight-inducing, and whatever Sid brought home from work for Priya to make for him was criticized and not appreciated. It was part of their negative interaction pattern in which Priya would begin her bid for an emotional connection, and Sid would walk away, further away emotionally, rather than solve their issues.
Emotional and Psychological Impact
The ongoing tension in Sid and Priya's relationship has left them both a great deal of emotional and psychological pain, fueling their frustration and feelings of isolation. Priya is single, bitter, emotionally exhausted, and is carrying the load of the home, children, and medical clinic that hardly anyone helps or acknowledges with Sid there. Her sense of frustration and abandonment is a physical and emotional absence. On the other hand, Sid feels under extreme pressure to be a good husband and father while performing his high-stress job. When Priya accuses him of being unavailable, he knows he is already doing his best for the family and feels guilty and inadequate. It only fuels his feelings of not being appreciated, and he pulls away rather than talking. At the top, their most prevalent communication pattern has been their pursuer with drawer pattern, which has driven emotional distance rather than intimacy. This also affects their children as the emotional undertones between their parents have also been present, and they have sensed the growing sense of disconnection within the family. In therapy, these deep emotional wounds had to come up so both of them could be aware of their repetitive patterns and learn to heal their traumatic emotions.
Second Section: Application of Emotionally Focused Therapy (EFT)
Overview of Emotionally Focused Therapy
Emotionally Focused Therapy (EFT) is a well-structured, evidence-based couples therapy based on attachment theory designed to rekindle emotional connection within relationships (Marren et al., 2022). A significant part of EFT is based on the information that partners' attachment needs and emotions regulate partner interaction. They enter into reactive patterns and create negative cycles without feeling emotionally connected to a partner. One partner will pursue connection using criticism or frustration, and the other will withdraw to avoid conflict, thus increasing emotional distance. EFT addresses a three-stage process to reorganize these patterns. The first stage is to identify and de-escalate negative cycles, and partners are assisted in seeing how their interaction pattern contributes to their distress. In the second stage, emotional responses are restructured as the therapist assists partners in being more emotionally vulnerable and responsive. In the third stage, these changes are consolidated as partners' new, healthier interactional patterns are reinforced, and emotional closeness is promoted. EFT is highly effective because it does not merely work on communication skills but also on the underlying emotional and attachment needs that are underlying relationship problems. EFT has resulted in long-term relational satisfaction by creating secure emotional bonds. Because Sid and Priya's problems were caused by emotional disconnection and not logistical disagreements, EFT was the ideal therapeut...
