Physical Activity and Heart Disease
Cardiovascular disease (CVD), as a disease, represents a real global threat as it kills millions of people every year. Thus, prevention is always necessary since CS comes as chronic and multicausal. Leisure-time physical activity (LTPA) is a form of physical activity described as cardiovascularly healthful at one time or another. Although this has been the trend with job-related physical inactivity or occupational physical activity (OPA), much research has not been conducted regarding its differential effects on CVD. Such publications include the systematic review by Kazemi et al. (2024). This paper evaluates the meta-analysis features, inclusion or exclusion criteria, researchers' approach, conclusions, and implications of the study for nursing practice.
Identifying the Article and Characteristics of a Meta-Analysis
Kazemi et al.'s (2024) study is a well-conducted meta-analysis where 103 prospective cohort studies were synthesized to examine the relationship between physical activity and CVD. Meta-analyses are research methods that statistically aggregate results from multiple studies, enhancing the reliability and precision of conclusions. Compliance with meta-analysis guidelines is strictly followed in this article, where many efforts are made to extract data from authentic databases, including PubMed. All the requirements for inclusion or exclusion of only quality papers are met. Similarly, the authors also applied dose-response meta-analysis based on linear and nonlinear models for the risk of CVD, Coronary Heart Disease (CHD), stroke, and atrial fibrillation (AF) based on the level of physical activity. To strengthen the methodological credibility of the conclusions, the authors used specific reliable methodological tools such as ROBINS-I to estimate the bias of the studies involved and GRADE to assess the quality introduced to the evidence. Cross-sectional random-effects models and restricted cubic splines analyses were applied to determine the relationships. Therefore, these methods enable the exclusion or inclusion of the article based on the set criteria to be included in the meta-analysis.
Inclusion and Exclusion Criteria
In this study, one can also see the applicability of Kazemi et al.'s (2024) meta-analysis. The authors achieved higher rigor by defining explicit inclusion and exclusion criteria. The criteria applied were prospective cohort studies of adults aged 18 and older with leisure and occupational physical activity exposure. Cardiovascular disease, coronary heart disease (CHD), stroke, and atrial fibrillation (AF) were all outcomes that were assessed. Therefore, those studies conducted on HF, with a retrospective or case-control design, less than 2 years of follow-up, or in athletes or patients with specific diseases were excluded. The selection process was systematic and thorough, and four peer researchers selected the study. And in case of a disagreement, the researchers had to discuss and come to a consensus. The studies included only those that provided hazard ratios (HRs) or equivalent risk measurements with sufficient participation rate and follow-up time. Secondly, when there was more than one report from the same group, the latest detailed one was selected. Such a process increases the reliability and accuracy of the meta-analysis results found in the study.
