Here was my post The crisis in nursing staffing has become a significant issue since the COVID-19 pandemic. As healthcare providers, we must identify, adjust, and address the needs in healthcare and the decreased staffing ratios due to the nursing shortage. The social determinants of the staffing crisis are increased stress and burnout among healthcare workers. Determinants of turnover related to managerial style and support are also factors. The inability to retain qualified nurses is detrimental to the delivery of the healthcare system and population health, as understood even before COVID. (Halter et al.,2017) Griffiths et al. (2021) compare staffing plans with higher and lower staff rosters and their impact on patient outcomes. Rosters that were left understaffed increase costs due to longer patient stays and pose a significant risk to patient safety. The nurses at my organization had been complaining for 6 months before the COVID pandemic struck regarding the unsafe conditions for patients related to staffing, and their complaints continued through the next year. They tried to convince the CEO of what was needed to improve to protect patients and prevent nurses from leaving. Ultimately, this led to a historic 301-day strike for safer patient care related to staffing ratios. Ten months after the strike commenced, the organization decided it would allow the striking nurses to return to their exact positions at higher wages, thereby resulting in staffing improvements and improved staff-to-patient ratios (MNA, 2022). Dixon-Woods et al. (2012) make many outstanding points in their narrative. I find challenges 9 and 10 especially relevant to my organization as it has not secured sustainability (challenge 9), and there is evidence of unwanted consequences (challenge 10). It has neither maintained the promised staffing ratios, resulting in continued poor patient care outcomes. Many of those picketing nurses have left and gone to another local hospital. According to those I speak with who did so, the staffing is better there, and “they pay ten dollars more an hour.” So, at the bottom, that shows that the healthcare system is just like any other economic marketplace. However, those same economic forces may not lead to sustainable wages and still result in rebound staffing shortages. In conclusion, the issue these nurses were fighting for has had a profound impact, leading to the deterioration of what many would have called, before the strike, an otherwise remarkable healthcare system. Both sides had to adapt their behavior to bring about improvements, but unfortunately, this was not achieved. The nurses remain burdened with heavy workloads, understaffing, and lack of experience due to the new hires, inevitably leading to more errors and increased patient harm. According to a study conducted by Andel et al. (2021), The consequences of inadequate staffing levels are severe, both in terms of patient outcomes and financial costs severely limit the length of patient stays, with increased risks contributing to patient safety, therefore raising the costs of healthcare organizations. By observing the implications of the current staffing deficit, our organization has noted the poor outcomes of patient care and the loss of professionalism as nurses leave to seek more satisfying working environments. To that extent, this paper will reflect on possible solutions by identifying strategies used by other healthcare institutions. Dixon-Woods et al. (2012) point to the need for recognition of the problem and its acceptability and the requirement of sound data gathering and tracking. To strengthen the capability for change at the organizational level, a culture of change needs to be created by addressing the staffing crisis as an emergent issue of significant concern to stakeholders to gain commitment to the proposed interventions.
