Name Tutor Course Date Topic 1: Physical Condition of Plant in Leadup Overall Disrepair A considerable amount of physical damage occurred at the BP Texas City Refinery during the disastrous event on March 23, 2005. This widespread disregard for equipment and infrastructure upkeep had a significant role in the tragedy's development. The refinery had seen ongoing underinvestment and cost-cutting efforts in the years preceding the catastrophe, which had led to a deteriorating operational environment (CSHIB 20). Business representatives disregarded or did not sufficiently handle important warning indicators of possible dangers. The systemic flaws in BP's approach to facility maintenance and safety procedures are reflected in the refinery infrastructure's failure to maintain crucial components.  Moreover, the infrastructure system in the Texas City facility had not been subjected to imperative upgrades as recommended by the Occupational Safety and Health Administration (OSHA) and other state and federal regulatory organizations. The safe removal from the operation of the refinery and the decommissioning of many essential systems, especially those connected to pressure relief and control, became impossible because many systems were old and not fit to accomplish this objective (CSHIB 66). In addition to stalling the development of new hazard prevention solutions, the failure to invest in the refinery's infrastructure renovation resulted in the catastrophic hazards.
