Barbara needs to make sure that she or she is informed, to screen and to deal with dual vulnerability in trauma and substance use. She is supposed to ensure that the purpose, risks, and expectations of the group are well defined and that group is the right group. According to the NAADAC Code of Ethics, workers must protect clients by making sure they are not harmed, working within competence, establishing a trauma-sensitive and culturally responsive environment in order to protect vulnerable participants (NAADAC, 2021).
Barbara needs to discuss boundaries of confidentiality, written group agreements and discuss expectations with a group periodically. Focus on respect to each other, safe records according to HIPAA, and no information identification in shared documents. Most of the clients will realize that confidentiality is not guaranteed in groups. Ethical practice requires teaching privacy expectations and group reinforcing of groups during treatment (ACA, 2014).
In case of breach, Barbara should take an immediate action, talk to the group about the consequences, strengthen the agreements, and consider harm-reduction measures. She should prepare a record of the incident and refer to supervisors where necessary and assist affected clients.
