Narrative
John is a 40-year-old accountant who looks unhygienic and, despite being cooperative, avoids eye contact. At first glance, John appears to be retarded due to his poor social skills. Before the sessions, he seemed agitated despite maintaining normal speech characteristics. However, he speaks slowly and sometimes with normal speed and volume. His affect was flat and mostly inconsistent with words. However, he seemed always aware of the time, place, and individuals around him. John described his mood as constantly down. Specifically, he reported feelings of worthlessness and hopelessness, feelings that emerged only after the divorce.
Despite issues with depression and alcohol abuse, his reports regarding delusions and hallucinations were consistent with what was observed. Interestingly, John reported pessimistic ideas about his condition, expressing no hope that his situation would get better. However, he fully understood and acknowledged his alcohol problem and attributed it to the stress from the divorce. While he denied suicidal intent and mental health problems, he acknowledged passive thoughts of self-harm that warrant monitoring. Based on his presentation, an integrated plan that addresses depression and alcohol abuse is needed. Cognitive behavioral therapy (CBT) is an ideal intervention that can help John improve his coping skills and recognize how alcohol affects his mental health.

Mental health problems pose a serious threat to personal well-being. As such, they require carefully planned and comprehensive interventions. In HUS professional settings, using assessments in diagnosis and treatment planning for mental health clients is important for various reasons. These include the assessments facilitating accurate diagnosis by identifying specific mental health conditions, differentiating between conditions, and identifying comorbidities. Additionally, the assessments help tailor treatment planning through personalized interventions and effective therapies (Bennett & Shafran, 2022). Lastly, the assessments help to monitor progress effectively by tracking improvement, identifying relapses, and measuring treatment effectiveness.
The merits of conducting MSE include that it facilitates the collection of objective and subjective data regarding the clientβs mental status. According to Volcevska et al. (2024), MSE is integral to mental health nursing since it is a core competency in acute inpatients, community mental health services, emergency departments, and health telephone triage services.
