BACKGROUND
Mission Viejo is a city located in Orange County, California. By 2017, the city had a population of approximately 96,016 individuals, where the males were about 49.3% and females 50.7% of the total population. The Mission Viejo Medical Center is keen and focused on ensuring that quality care is provided to all city inhabitants. Additionally, the facility provides a wide range of medical specialties, and the medical center is dedicated to providing quality care across Orange County. The institution is surrounded by other medical facilities that include Mission Hospital Regional Medical Center and Children’s Hospital at Mission. Some of the conditions prevalent in the city include asthma, diabetes, heart disease, mental health, and cancer. Some of the unique illnesses witnessed in the city include Lyme disease, which may be caused by ticks, and some viruses cause respiratory illness. This literature examines and explains the need for Mission Viejo Hospital to construct a new hospital wing to meet the senior population's health needs.
Besides, about 15% of the population in Mission Viejo are seniors, and nearly 22% of the seniors in the city live alone (SeniorCare, 2020). However, the majority of seniors live with their families. Some of the facilities that serve seniors include assisted living facilities, adult day care centers, home health care services, and nursing homes. Some care services offered to seniors include care for prolonged illnesses and disabilities, and nursing homes may help the population to recuperate from long-term diseases such as surgery and chronic conditions such as cancer. The length of stay of seniors in hospitals for most individuals is five years or more.

Common medical conditions
Ideally, the increase in the aging population has increased the pressure of states to improve their healthcare services so that they can meet the need of this population. With the high percentage of seniors, healthcare facilities are required to set aside more resources to cater to the aging population's health needs. Aging has been identified as a primary risk factor for various common diseases (Swenson, Bastian, & Nembhard, 2018). There are numerous medical conditions common among the elderly; these include chronic diseases like diabetes, cancer, and cardiovascular diseases. Also, cognitive health problems and physical injuries are high in the old population.
Medical institutions for seniors
There are different medical facilities in the city that are responsible for taking care of the seniors. These may include the assisted facilities where the seniors are provided with daily personal care; they can also receive therapies. Besides, the nursing homes are also instrumental in facilitating an environment that is ideal for meeting the needs of elderly patients and also act as the right place where the seniors can socialize and stay. Moreover, the adult daycare centers are available, creating an environment where the seniors can participate in planned group activities that facilitate socialization and promotion of the patients. Besides, these facilities are also crucial in taking care of elderly individuals who have been isolated and lonely.

The city has a population of over 96,016 people, where about 49.3 percent are men, while 50.7 percent are women. Ideally, the median resident age in Mission Viejo is about 44.7 years as compared to that of California, which is 36.5 years. Additionally, the median household income is approximately $102,787, which is an increase from $78,248 in 2002. The city has people from various races, with the Whites constituting about 65.2 percent of the population. Seventeen percent are Hispanics, Asians are 10.9 percent Blacks 1.4 percent, and individuals with mixed races form 4.2 percent of the city's population and the seniors' from about 15 percent of the city.
MARKET RESEARCH AND SEGMENTATION
Based on the data gathered, the target market's subjects are elderly individuals in the city. Most importantly, the seniors are located in different nine municipalities, including Aliso Viejo, Ladera Ranch, Laguna Hills, Laguna Niguel, and Laguna Woods, among others. Additionally, the seniors require various special needs, such as counseling; this is important because some of the elderly are isolated and do not close individuals to look after them. There are several health services that the elderly would wish to acquire; these include having access to mental healthcare because aging is linked to mental health issues. Also, people would want access to healthcare for chronic conditions such as heart problems. To meet the healthcare needs of the seniors in the city, the facility would require services for specialists such as cardiologists and psychologists. The seniors prefer center medical hospital where they have dedicated and trained specialists to take care of them and offer the needed services.

ORGANIZATIONAL STRUCTURES AND FUNCTIONS
The city of Mission Viejo, located in Orange County, California, has a relatively large population of seniors and is determined to develop a hospital wing intended to take care of the older adults from the city and other surrounding municipalities. The establishment of the hospital is ideal because seniors are likely to experience different health complications, including prolonged illnesses, chronic conditions such as cancer, and heart complications. Additionally, the hospital's development would help manage the long-term diseases common among the elderly. However, the hospital will only be capable of proper organizational structures. Its functions are implemented to ensure that Golden Age Hospital and community clinics can share critical resources such as human labor, nurses, and other practitioners.
Common services or functions
Ideally, there are different services that the proposed Golden Age Hospital can share with the community clinic. To ensure that the new hospital operates and records the intended objectives, it is critical to make sure that the facilities share the organizational structure where the health centers would have the same management levels. Additionally, to ensure efficient control and management of the hospital, it is critical to address the issues related to complexities associated with the chain of command, thus creating a conducive environment for facilitating better care delivery (Gougeon, Johnson & Morse, 2017). This would be made possible by ensuring that GAH and the Community Clinic share some resources such as the pharmacy and operating rooms. Such measures would aid in minimizing the cost of running and managing health facilities. This would be made possible through a collaboration plan where the professionals and specialists would be made possible to attend to patients in both centers.
Organizational relationships
The Golden Age Hospital and the Community Clinic are required to develop and implement effective and reliable relationships that can promote their success in effectively meeting the health needs of the target population. Additionally, a clear and concise organizational structure is fundamental in determining how tasks and medical responsibilities are shared between the facilities to ensure that practitioners from the hospital and the community center offer medical care to the seniors. It is critical to note that a practical organizational framework would significantly impact reporting between practitioners from both facilities, which would impact the quality of care offered to the elderly population (Edwards, 2014). The relationship between the two facilities can exist at the vertical level, which determines the interactions between executives and supervisors and may also be horizontal, thus influencing interactions between the subordinates and other teams.
Integration of services offered by physicians
The main goal of integration is to ensure that professionals from both facilities are involved in care delivery. Therefore, to ensure that the services offered by physicians in geriatric treatment in both Golden Age Hospital and Community Center are effectively integrated, a collaboration plan should be developed to help specialists in both facilities attend to patients in both centers. Integration would help establish good relations between the health care systems from both, and integrated services would benefit the seniors with complex health problems (Everink et al., 2014). Besides, the integration and collaboration of healthcare professionals from the two health facilities would increase the quality of care offered to the city's aged population.
Management aspects
Effective management of the facilities may determine the success of care offered to elderly individuals in the city of Mission Viejo and other municipalities. Most importantly, well-organized services would help improve the quality of services provided to the intended group of patients (Griffith & White, 2005). Furthermore, adequately managed facilities would create an environment that can encourage proper planning, controlling, and organizing. All critical stakeholders, including the care providers, would be involved in dealing with what is happening within the facility.
Organizational forms and mission statement
The most appropriate organizational method that would be established between the Golden Age Hospital and the Community Center is "Open" Physician-Hospital Organizations (OPHOs) and would make it possible for the health facilities to promote managed care through increasing the availability of administrative response by the physicians (Cuellar, Gertler, 2006). Besides, having an ideal mission of both the hospital and the community center can determine the direction of the health facilities. It would also influence the success of services offered to support elderly patients. The mission would highlight the two facilities' determination to facilitate the provision of high-quality healthcare and other critical supportive services to promote the well-being of the elderly population.

The four facilities will be organized in the order presented above and the directors will be selected from all for facilities. Additionally, only the CEO of Mission Hospitals is connected to the four departments because the space is limited. However, it should be noted that the entire organizational diagram will be completed when all CEO should also be connected to all departments. This organization indicates that the CEOs from the various facilities will be directly involved in what is happening in all the departments.
Recommended hospital size and primary treatments
The suggested number of hospital beds for the newly developed hospital is about 200 beds. However, there is a need to note that the beds should be distributed depending on the space available. This is critical because the facilities' goal is to ensure that all older adults are provided with quality care services. The primary treatments offered in the facility will include cancer treatment, diabetes, cognitive health problems, cardiovascular conditions, and malnutrition.
Physician-Hospital integration
Ideally, improving the integration between the facility and the care providers is critical in promoting health services and would also improve care delivery.
