The city is determined to construct a hospital wing to take care of old patients. Mission Viejo Medical Center is located in Mission Viejo, California, and has two geographically adjacent institutions, Mission Hospital Regional Medical Center and Children’s Hospital at Mission. It is suggested that the Golden Age Hospital should be built in Mission Veijo Hospital to take care of the elderly population. It is essential to note that the construction of this hospital will also serve other seniors in other municipalities.

Aging has been determined as a critical risk factor for different common diseases; this means that older adults are more likely to be affected by a wide range of conditions (Swenson, Bastian, & Nembhard, 2018). The increasing number of elderly individuals necessitates changing healthcare policies and practices to meet their needs. Besides, hospitals are required to set more resources so that they can meet the healthcare needs of the seniors. There are various medical conditions that are likely to affect seniors; these include chronic conditions such as diabetes, cardiovascular diseases, and cancer (Jaul & Barron, 2017). Also, the elderly population is likely to develop cognitive health problems, as well as physical injuries.

There are several medical institutions that can take care of elderly patients; these include the assisted facilities where they are provided with daily personal care. Also, nursing homes create an environment where the seniors can stay under the watch of trained professionals, and they can socialize with other seniors. To ensure that the services offered by physicians in geriatric treatment in both GAH and CC are effectively integrated, the professionals should be provided with a collaboration plan that would make it possible for specialists in both facilities to attend to patients in both centers. Integration would help in establishing good relations between the health care systems in GAH and CC. Integrated services would be to benefit seniors with complex health problems (Everink et al., 2017).

OPHOs can also manage other facilities that be considered critical to elderly patients, such as ambulatory.

The mission of GAH and Community Clinic should be to facilitate the provision of high-quality healthcare and other critical supportive services to promote the well-being of the elderly population. Ideally, the facilities should be focused on treating the conditions prevailing among the seniors and also help in reducing social isolation among this population.

Although the suggested number of beds in the hospitals should be about 200 beds, it is essential to ensure that the maximum number of beds should be established to help in meeting the needs of a broad group of deserving older adults. Also, the number of beds is proportional to the size of the facility.

The common treatments to be treated include diabetes, cancer, cognitive health problems, cardiovascular conditions, physical injuries, and malnutrition. Additionally, physician-hospital integration will be critical in increasing the efficiency of the health facilities in meeting the needs of the elderly population in the Mission Viejo area.

Having a tighter integration of physicians with the health facilities would help in improving care coordination.
