BACKGROUND
Mission Viejo is in Orange County, California, and has a population of approximately 96,000 people. The city has about 48662 women and 47,354 men. The city’s median household income is about $694,600, which is an increase from $78,248 in 2000. About 15% of the city’s inhabitants are seniors. The city is determined to construct a hospital wing to take care of old patients. Mission Viejo Medical Center is located in Mission Viejo, California, and has two geographically adjacent institutions, Mission Hospital Regional Medical Center and Children’s Hospital at Mission. It is suggested that the Golden Age Hospital should be built in Mission Veijo Hospital to take care of the elderly population. It is essential to note that the construction of this hospital will also serve other seniors in other municipalities.
EXTERNAL ENVIRONMENT-DATA GATHERING
The city of Mission Viejo has over 96,000 people, where women form 50.7% of the population, while men comprise 49.3% of the people. The city’s population has increased by 3.1% since 2000. Additionally, seniors form 15% of the city’s population (SeniorCare, 2020). The city also comprises various races, including the Whites, who are approximately 65%, Hispanics (17%), Asians (11%), and Blacks (1.4%). Also, 4.2% of the population is made up of mixed races.
Population in municipalities
All the nine municipalities have high number people; Aliso Viejo-51,671, Ladera Ranch- 22,980, Laguna Hills-31,318, Laguna Niguel-66,334, Laguna Woods-16,200, Lake Forest-84,293, Mission Viejo-96,016, Rancho Santa Margarita-48,793, San Juan Capistrano-36,064. The most populated municipalities are Mission Viejo and Lake Forest and would have a high number of seniors requiring care.
Common medical conditions
Aging has been determined as a critical risk factor for different common diseases; this means that older adults are more likely to be affected by a wide range of conditions (Swenson, Bastian, & Nembhard, 2018). The increasing number of elderly individuals necessitates changing healthcare policies and practices to meet their needs. Besides, hospitals are required to set more resources so that they can meet the healthcare needs of the seniors. There are various medical conditions that are likely to affect seniors; these include chronic conditions such as diabetes, cardiovascular diseases, and cancer (Jaul & Barron, 2017). Also, the elderly population is likely to develop cognitive health problems, as well as physical injuries.
Medical institutions for seniors
There are several medical institutions that can take care of elderly patients; these include the assisted facilities where they are provided with daily personal care. Also, nursing homes create an environment where the seniors can stay under the watch of trained professionals, and they can socialize with other seniors. The adult daycare centers create an environment where the clients can take part in planned group activities.
Local (Mission Viejo area) City demographics
The city has approximately slightly over 96,000 people with a higher number of women than men. The median resident age is about 44.7 years and which is higher than the state of California median resident age, which is approximately 36.5 years. The median household income has increased significantly since 2002. Besides, the city has different races, including whites, Hispanics, Asians, and Blacks, among others.
Direct market research and segmentation
The data collected is bases on the number of older adults in the city of Mission Viejo from the municipalities. This information is required in making decisions about the construction of the new hospital wing, which is dedicated to elderly individuals. The resources available should be located depending on patient’s needs (Mele, 2008). The seniors are in need of different services, including counseling, mental health, and other chronic conditions such as heart disease.
Survey Questionnaire
The questions included in the survey will help comprehend the services that the potential patients would want to be made available in the facility. The questions should be aimed at evaluating whether the special facility is needed, whether there are unmet health needs, whether the seniors encounter challenges when receiving care, and if services offered in nursing homes are better than home care, among other critical issues.
ORGANIZATIONAL STRUCTURES AND FUNCTIONS
Common services or functions
There are various services that the proposed Golden Age Hospital can share with the community clinic; these may include the organizational structure where the health centers would have the same levels of management. Besides, this would be critical in allowing efficient control and management and would also help in dealing with problems that are related to understanding the chain of command. Additionally, GAH and the Community Clinic would share their pharmacy and operating rooms, which would be ideal in reducing the cost of running and managing the health facilities. This is possible when the professionals are provided with a collaboration plan that would make it possible for specialists in both facilities to attend to patients in both centers.
Organizational relationships between the structure blocks
The organizational relationships established between the organizations is critical in promoting the achievements of the organizational goals that Golden Age Hospital and the Community Clinic have in an efficient manner. The established organizational structure would help in determining how the tasks and responsibilities of taking care of the elderly patients are assigned following formal reporting relationships between the two facilities (Edwards, 2014). Therefore, the relationships of the facilities will exist at two levels vertical and horizontal. The vertical linkage will determine the interaction between the supervisor and the subordinates. Horizontal linkage will influence interaction task forces and other teams.
Integrate the services of physicians
To ensure that the services offered by physicians in geriatric treatment in both GAH and CC are effectively integrated, the professionals should be provided with a collaboration plan that would make it possible for specialists in both facilities to attend to patients in both centers. Integration would help in establishing good relations between the health care systems in GAH and CC. Integrated services would be to benefit seniors with complex health problems (Everink et al., 2017).
Management aspects
Additionally, the management of the facilities would be critical in promoting the quality of services offered to the elderly population (Griffith & White, 2005). Ideally, an excellent organization of GAH and CC would facilitate long-term results that would satisfy all stakeholders. The management aspects that must be addressed to boost the operations and success of the facilities include planning, organizing leading, and controlling, thus ensuring that the health facilities are operating as expected.
Organizational forms
“Open” Physician-Hospital Organizations (OPHOs) is the most applicable organizational form that can be established between the GAH and CC. Ordinarily, this form helps in facilitating managed care and can also help in ensuring that administrative are made available to physicians (Cuellar, A., Gertler, P, 20006). OPHOs can also manage other facilities that be considered critical to elderly patients, such as ambulatory.
The mission statement for the GAH and, Community Clinic
The mission of GAH and Community Clinic should be to facilitate the provision of high-quality healthcare and other critical supportive services to promote the well-being of the elderly population. Ideally, the facilities should be focused on treating the conditions prevailing among the seniors and also help in reducing social isolation among this population.

The four facilities will be organized in the order presented above. Only the CEO of Mission Hospitals is connected to the four departments because the space is limited. However, it should be noted that all other CEO should also be connected to all departments. This organization indicates that the CEOs from the various facilities will be directly involved in what is happening in all the departments.
Recommended hospital size
Although the suggested number of beds in the hospitals should be about 200 beds, it is essential to ensure that the maximum number of beds should be established to help in meeting the needs of a broad group of deserving older adults. Also, the number of beds is proportional to the size of the facility.
Major services/treatments
The common treatments to be treated include diabetes, cancer, cognitive health problems, cardiovascular conditions, physical injuries, and malnutrition. Additionally, physician-hospital integration will be critical in increasing the efficiency of the health facilities in meeting the needs of the elderly population in the Mission Viejo area.
Physician-Hospital integration
Having a tighter integration of physicians with the health facilities would help in improving care coordination. The alliance would promote a robust physician-hospital association that would facilitate geographic reach, quality management, and organized oversight (Satiani & Vaccaro, 2010). There are, however, factors that may affect the integration between the physicians and the health facilities. For instance, reimbursements are often decreasing, and the expenses are on the rise. Such forces make the regulatory environment complicated, and this is likely to make physicians want to look for other means to obtain income security. In some situations, physicians are unwilling to work in an environment where they are highly controlled.
FINANCE AND BUDGETING
The construction of hospitals for the elderly population in the city of Mission Viejo will be essential in responding to the health needs of the older population in the city and the surrounding municipalities. The hospital is facilitated by the need to meet the health requirements associated with older people, such as chronic diseases, prolonged illnesses, and cardiovascular conditions, among others. Effective coordination of the hospital is critical because it will help in promoting engagement with the community clinic and would also improve the quality of care offered to the patients. Also, proper coordination of the hospital will promote proper financing and budget to facilitate the hospital’s financia...
