Introduction
Control Your Well-being Project is a health and wellness program that seeks to improve the quality of life and health amongst those living with diabetes and hypertension through health education. Control Your Well-being Project is focused on the health and wellbeing of a person from a holistic perspective. Program Description and History
Control Your Well-being Project accounts for current and past history associated with increased challenges in managing hypertension and diabetes. The two chronic illnesses, which may often coexist, have been historically problematic due to their increasing prevalence, increased healthcare costs, rising morbidities, and mortality. Management of noncommunicable diseases such as diabetes and hypertension need consistent access to drugs, laboratory and diagnostic services, data, qualified and trained healthcare personnel, and an educated and well-empowered population (Correia et al.,2019). Control Your Well-being Project will strongly undertake collaborative patient education and link patients to the healthcare system for further consultations, treatment, referral, and diagnostic services. Control Your Well-being Project goals will form part of primary healthcare by preventing and helping control morbidity, premature death, and disabilities occasioned by diabetes and hypertension.
The proposal of this project is informed by significant gaps in the management of diabetes and hypertension. Even though there is significantly high awareness and knowledge about hypertension and diabetes, unhealthy lifestyle practices and irregular screening are frequent (Sina, Graffy & Simmons, 2018). The findings by Saleh, Ara & Afnan (2016) show that type 2 diabetes patients lack adequate knowledge on the understanding of diabetes, its predisposing factors, the blood glucose target levels, approaches of dealing with hypoglycemia, proper dietary practices, and basic approaches to foot care. There is sufficient evidence to show that patient education and ensuing lifestyle have immense gains in diabetes care, but the evidence is less in the context of enhancing hypertension care (Hassanein et al.,2021). Thus, there is a need to boost access to adequate, timely, accurate, and quality information about hypertension and diabetes that would assist the patient and community, in general, in adopting healthy behaviors. Control Your Well-being Project seeks to bridge these gaps by availing health education that reinforces appropriate behaviors that would help people cope with diabetes and hypertension. Education, the patients on ways of preventing and dealing with hypertension and diabetes, forms the primary activities of the Control Your Well-being Project.
The overall goals and outcomes of the Control Your Well-being Project are to strengthen diabetes and hypertension prevention efforts and to provide practical strategies to the patient with these diseases on optimal ways of coping with the disease process. These goals are measurable. Control Your Well-being Project will form groups that will engage in self-management approaches to controlling diabetes. Control Your Well-being Project will not work like the conventional education program that imparts disease-specific knowledge about exercise, diet, diagnostic tests, and medication but will engage in creating platforms that provide health information that will improve durable compliance to disease management and prevention strategies. Control Your Well-being Project is geared towards harnessing the motivational, emotional, and inspirations aspects to ensure acceptance of hypertension and diabetes, thereby leading to the promotion of coping strategies.
Stakeholder Engagement
Control Your Well-being Project will involve various stakeholders in assessing this plan in terms of need assessment, overall objectives, and strategy. Stakeholders represent people and organizations interested in this project and will invest and support the project. These stakeholders will actively participate in all evaluation stages of the program.
The first class of stockholders includes those who will be affected by the program. This category can involve hypertensive and diabetic patients or those at greater risk of these chronic illnesses, community members, and advocacy groups. The Control Your Well-being Project primarily targets hypertensive and diabetes patients and those who have a significant health risk.
