Documentation includes writing case notes, clinical assessments and reports, and referrals. Having this skill is very important for legal liability, continuity of service, and funding justification. Records in social work are legal documents that need to be objective, factual and complete. If they are not clear, missing, or subjective, it might weaken cases in court or during an audit.
