Even though the symptoms are bothersome, he is more concerned about his low back and hip pain which began approximately one month ago which he initially attributed to lifting heavy boxes. The pain is now constant and disrupts his sleep. Labs reveal a normal urinalysis and CBC, a PSA of 7.2. A DRE reveals an enlarged, nodular prostate. The x-rays of the LS spine are positive for mild degenerative changes and what appears to be a cystic mass near the spine. An understanding of the factors surrounding women’s and men’s health can be critically important to disease diagnosis and treatment in these areas. This importance is magnified by the fact that some diseases and disorders manifest differently based on the sex of the patient. Effective disease analysis often requires an understanding that goes beyond the human systems involved. The impact of patient characteristics, as well as racial and ethnic variables, can also have an important impact. An understanding of the symptoms of alterations in systems based on these characteristics is a critical step in diagnosis and treatment of many diseases. For APRNs, this understanding can also help educate patients and guide them through their treatment plans. In this Assignment, you examine a case study and analyze the symptoms presented. 1 What are the factors that affect fertility? Sexually transmitted diseases (STDs) are implicated in female infertility, majorly through their effect on fallopian tubes. Infections from N. gonorrheae and C. trachomatis are predominant etiologic factors of pelvic inflammatory disease (PID). The latter is considered a vital cause of tubal factor infertility (TFI). These infections may spread to the fallopian tubes, causing salpingitis, which is an inflammatory process that can destroy and block the fallopian tubes, thus preventing the eggs from entering and traveling through them, significantly reducing the possibility of conception (Tsevat et al., 2017; Jennings & Krywko, 2022). Other factors include ovulatory dysfunction and menstrual cycle problems, structural abnormalities of the female reproductive system. These masses cause obstruction or failed implantation, such as endometriosis or uterine fibroids, autoimmune diseases, hormonal factors, and polycystic ovarian syndrome (PCOS) (NIH, 2017). Conversely, factors affecting male fertility include hormonal factors such as problems with the hypothalamic-pituitary axis, like Kallmann syndrome and hypogonadotropic hypogonadism, physical conditions that result in an obstruction such as varicocele results to enlarged veins that prevent the sperm from traveling through epididymis or testicular torsion which can adversely affect sperm production and quality, sexual problems like erectile dysfunction, chromosomal abnormalities like in Klinefelter syndrome, and problems with gene expression (Babakhanzadeh et al., 2020). 2 Why do inflammatory markers rise in STD/PID?
