Given the high INR, first, omit one to two dosages of Warfarin, as the INR is monitored to prevent bleeding. Offer 1-2mg of vitamin K once (Takase et al., 2018, pp.1-2). Once the IRN is within the range of 2-3, adjust the Warfarin dosage to 3.5mg PO/IV qDay for seven days while monitoring INR and maintaining the Amiodarone dosage (Farinde, 2024). Scenario 2 Protein binding impacts the bioavailability and dissemination of compounds by restricting the drugs’ capacity to pass across membranes and barriers in the body (Wanat, 2020, pp. 3221-22). The medications with higher proteins bound compete with the rest for the same binding site, reducing their pharmacological activity (Bohnert & Gan, 2013, p.2955). Managing the patient’s medication would begin by reviewing and assessing the protein binding capacity. Blood tests of the three drugs would establish which leads to competitive displacement while the rest float free (Charlier et al., pp. 1209-10). If you develop significant displacement by one drug, the dosage can be decreased, or the timing of their administration can spread through the day to limit competition.
