As the country focuses on the restructuring of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and fewer will be available in acute care hospitals.
Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
A minimum of three scholarly references are required for this assignment.

The nursing profession is one of the most flexible and diverse in the healthcare field. The healthcare system is under a continuous process of change, which ensure that the matters arising, in terms of challenges and improvements, are addressed. This is especially the case with the ever-changing economic environment, and the dynamic nature of the population. The changes in nursing practice, focuses on a number of aspects that make it holistic, such as nursing leadership, education and professional practice.
The role of the nurses in the process of exacting changes in the sector is as crucial as the changes themselves. Of more importance however, is the constant assurance that there is the continuous availability of nursing jobs in any capacity. Even though it has been subject to much political debate amidst calls for amendment, the 2010 Affordable Care Act had placed much responsibilities upon the medical personnel, and therefore helped create a system that focused more on the patient than ever before (Fisher et.al. 2009).
Continuum of Care
Also known as continuity of care, it is the relationship that exists between the patient and the medical practitioner handling him or her. It goes further than just treatment of the medical issue at hand. Continuity of healthcare is a vital pointer to an efficient healthcare system. Whereas there might be very little that can be hinged upon it as a single entity, there are other contributors to, such as coordination and integration of care as well as client-centered care (Hussey et.al. 2009). The two main elements of continuum of care are; the long-term nature of care and its continuity, and the particular medical details and needs of the patient.
