Introduction
Pressure ulcer is one of the leading preventable conditions, and 60,000 patients die from the condition as a secondary diagnosis in the US (Antonetti, 2012). Additionally, patients with pressure ulcers are three times more likely to stay longer in hospitals in comparisons to other chronic diseases. Nursing interventions focus on dealing with the leading contributors of pressure ulcers including nutrition, moisture, pressure and friction/ shear. However, educating patients and nurses has the potential to drastically reduce the prevalence of pressure ulcers, especially when there is tailored education in line with patients needs. This paper highlights on a prevention pressure ulcers capstone project, it also touches on details about the setting, description of the problem, impact of the problem and proposed solution to reduce cases of pressure ulcers
Setting/ context
The setting of the prevention pressure ulcers can be observed in hospitals for patients in long-term care. Bedridden patients in long term facilities, those with impaired mobility and who have recently undergone surgery are at a high risk of pressure ulcers (Thomas, 2001). Hence, there is a need for effective skin care procedures in hospitals to prevent pressure ulcers, besides using repositioning on good quality mattresses. The values of nurses on skin care could be a determining factor in prevention of pressure ulcers in hospitals. In such instances where there are high incidences of pressure ulcers, careful attention is necessary to reduce such incidences (Thomas, 2001).
Detailed description of the problem/ issue or education deficit
Pressure ulcers typically occur when there is skin breakdown upon application of pressure, and the bed sores of pressure and friction around bony prominences. The pressure ulcers may occur in a short period of time because of great force being exerted or it may occur over a longer period of time whereby there is impaired circulation. In such cases, when there is inadequate circulation there are risks of tissue necrosis and ischemic lesions which may in turn cause infection. Knowledge on risk factors helps to reduce incidences of pressure ulcers, with lack of time and knowledge on preventive measures being barriers to prevention and management of pressure ulcers (Strand & Lindgren, 2010).
Impact of the problem
Nursing determines the quality of care, and since patients with pressure ulcers require more attention, the condition has a negative impact on nursing staff. Essentially, nurses require spending more time managing pressure ulcers and at times other areas receive less attention. However, since nursing staff cannot adequately improve practice, this has a direct impact on nursing safety and also affects the health care delivery system (Donna, 2013). With increased attention on the management and prevention of pressure ulcers, there is a high likelihood that there will be positive patient outcome. Training and educating nurses to focus on skin inspection, nutrition, hydration and repositioning is critical to improve the patients’ health (Donna, 2013).
