Introduction
To improve health care outcomes within a community, leaders must understand the needs of specific populations. In addition, it is important that leaders are able to gather and present persuasive evidence to decision makers to obtain the necessary funding and establish these needs as priorities for budget and resource planning. Thus, proposed changes should support improved outcomes and align with the current or needed health care policy.
This assessment provides an opportunity to propose a community health care change that is based on the community health assessment you completed in Assessment 2.
Preparation
Your community health assessment was well received by the executives at Vila Health. Consequently, they have asked you to recommend a change in the community that will improve health outcomes. They have also asked you to submit your recommendation in a formal change proposal. You are confident that you can present a strong case for change, including a financial plan, that will ensure the success of this initiative.
In this assessment, you will draft a change proposal to executive leaders to solicit support for a change to the current community health care system. To prepare for the assessment, you are encouraged to begin thinking about your justification for establishing the recommended change as a priority, a strategy for leading change, and how the change will be funded. Complete this assessment in two steps:

2. Draft a change proposal to justify your recommendation for improving the health of the community and establish the change as a priority for the next budget cycle. Consider the feasibility of your proposed change, in terms of overall scope, funding, and timeframe.

A community health assessment identifies potential health risks in a given population, which increases understanding of the population's medical needs. Environmental Analysis
Based on the windshield survey, environmental conditions play a significant role in the quality of health care for the residents of Jordan. According to the World Population Review (2024), the town has a population of 6,885. The town has local churches, schools, and medical centers that cater to this population. St. Francis Regional Medical Center caters to the medical needs of the residents of Jordan. The medical center has rooms to cater to Jordan's residents' inpatient, outpatient, and emergency services, promoting timely and efficient service. The institution aims to increase health equity and promote quality patient care services (Dixon, 2021). St. Francis has a mental health department that promotes mental well-being and a full continuum of mental health services for patients. The substance abuse department offers support for individuals with substance disorders, promoting prevention and recovery for residents in Jordan. Residents are individuals from diverse cultures; hence, the center promotes culturally responsive care through respecting and appreciating the different cultures (Villa, n.d). The staff are trained to handle the patient respectfully; hence, they promote culturally responsive care to the residents.
The parks and recreational facilities still need to be fully utilized in Jordan to promote quality healthcare outcomes. The veteran's park encountered a funding cut that negatively impacted the residents. The Park has turned into a place where children engage in activities such as substance abuse. The organized events have drastically reduced, making the environment conducive for children to litter with beer cans and smoke since minimal supervision exists.
