The conversation script is based on the interactions between the Psychiatric-Mental Health Nurse Practitioner (PMHNP), the mother and her toddler as part of a comprehensive psychiatric examination. I engaged in a dialogue with the toddler, ensuring that it fits her developmental stage. I also included play scenes into the interaction as toddlers enjoy placing play figures into airplanes or cars and then going places. The following are the questions asked to the toddler and the subsequent responses.
PMHNP: How old are you?
Toddler: I am 3 years old.
PMHNP: Please tell me what happened when mum picked you up, and you left dad?
Toddler: Dad first assured me that he would come back soon. I cried as dad left. He said goodbye with a kiss and waved at me. I’m worried dad has taken a long to come back.
PMHNP: Who will travel on this airplane?
Toddler: Mom.
PMHNP: Where will you travel to?
Toddler: I think we should visit dad.
PMHNP: Would you travel to Auntie Maria’s place?
Toddler: (Reluctantly) Yes, but I wouldn’t say I like when she comes to our place.

Toddler: I feel worried and unhappy most of the time, and I do not feel like talking to people anymore. I also stay awake at night, fearing that I will have nightmares when I fall asleep. I find it difficult sleeping without my dad nearby.
PMHNP: What makes you afraid?
Toddler: I am constantly afraid of unfamiliar individuals and places, especially when my dad is away. I am also worried about whether dad will get back home or not. I fear that something terrible might happen to me when dad is away. I don’t like people visiting our home.
PMHNP: When did you start getting worried?
Toddler: I started getting worried when my father was recalled to naval duty, leaving me behind. I am used to staying with dad. Mom is not always available like dad; she is busy at work.
PMHNP: Where do you live and with whom?
Toddler: I live in San Diego with mom.
PMHNP: What makes you happy?
Toddler: I love visits to the park and watching cartoon videos.
PMHNP: What do you think about your breathing?
Toddler: It’s a relaxing experience to breathe. I have difficulties breathing have taking eggs. My mom says eggs affect my ability to breathe. I don’t think about the breathing issue since it only happens after taking eggs.
