Topic: Medication Study Guide: Risperidone
Introduction:
Risperidone is an atypical antipsychotic mostly utilized to treat schizophrenia, mania, and psychosis. This drug is a second-generation antipsychotic medication. Reports show it is one of the most widely utilized SGAs (FDA, n.d.). This guide offers extensive information on Risperidone, including its uses, dosing, side effects, and significant considerations for healthcare providers.
Description:
* Brand Name: “Perseris, Risperdal, Rykindo, Uzedy”
* Generic Name: Risperidone
Link: https://www.drugs.com/risperidone.html
FDA Indication Uses:
* Risperidone is specified for the treatment of “schizophrenia and irritability” related to autistic disorder (Drugs.comLinks, 2023). It is also designated as “monotherapy, or adjunctly with lithium or valproic acid, for the treatment of acute mania or mixed episodes related to bipolar I disorder” (FDA, n.d.).
Non-FDA Uses:
* It is also used off-label for different conditions, such as generalized anxiety disorders, dementia, attention-deficit hyperactivity disorder, depression, and insomnia, among others (FDA, n.d.).
Drug Classification:
* Atypical antipsychotic (Second-generation antipsychotic)
Structure:
Mechanism of Action:
* Even though its precise mechanism of action is not fully understood, existing action is on the antagonizing dopamine D2 receptors and serotonin 5-HT2A receptors, which helps in reducing symptoms of psychosis and stabilizing mood. For instance, schizophrenia is believed to result from an excess of dopaminergic D2 and serotonergic 5-HT2A activity, leading to overactivity of central mesolimbic pathways and mesocortical pathways, respectively.

Pharmacokinetics:
* Distribution: Well distributed throughout the body, with a volume of 1-2 L/kg.
* Metabolism: Hepatic, via CYP 2D6, an “enzyme that catalyzes the hydroxylation of Risperidone to 9-hydroxyrisperidone” (Drugs.comLinks, 2023).
* Absorption: Well absorbed orally and can be administered with or without meals.
* Excretion: excreted in urine and feces. In older adults, renal clearance is reduced, and exclusion half-lives are extended compared to young, healthy individuals.

Pharmacodynamics:
* The onset of action is within 1-2 hours
* Peak plasma concentration: 1-2 hours for Risperidone, 3-4 hours for 9-hyroxyrisperidone
Dosing and Administration
* General Dosing:
* Schizophrenia: Begin at 0.25 mg/day, increase ...
