Description
The entire experience mainly entailed the assessment of a resident who had recently been admitted to long-term care. The resident had a DNR code and lived at home with her husband. She had a lot of challenges that were further compounded by the fact that her husband was also struggling with certain medical conditions. Some of the challenges she had included poor medication management, inability to identify her pills, inability to take administered medication to herself, frequent falls, and hyperglycemia. Aside from the above, the resident did not have any close relatives, children, or family that would take care of them, and therefore, when challenges compounded, it was left to the husband to help. Roberts and Ishler (2018) indicate that the presence of family and their meaningful involvement in patients admitted to long-term care helps to enhance their quality of life. However, for the resident in question, the lack of familial presence meant that she was devoid of the care and concern that she would otherwise have had.
Throughout the assessment session, I felt pity for the resident because of her deteriorating condition, the husband’s deteriorating condition, and the lack of familial presence in their lives. While it may seem unprofessional, I felt helpless knowing that the resident would likely be back with injuries from falls or hyperglycemia because of medication mismanagement. However, I also felt the lack of fight in the resident. While it may seem strange at the moment, I felt that the resident had given up the fight and was simply making peace with herself, waiting for her day with fate. While it is easy to claim that the DNR was a clear indication, something within the resident felt unmoved and unhinged from the help I was dispensing.
Reflection
The assessment of the resident was quite illustrative as it revealed some of the challenges she is facing while at home. First, it became evident that she needed to relocate to a nursing home where she would be taken care of and closely monitored. The mere fact that her husband was also in need of long-term care made it increasingly crucial that it be insisted that she moves to a nursing home. Soon, it would be impossible for the husband to offer the little help he has been able to offer. While the PSW is indeed playing a crucial role in helping the resident, the lack of governance and skills standards for PSWs means that they are not as qualified for the task at hand (Hopwood & MacEachen, 2021).
