Intro
The project entails pain management in surgical patients. Controlling postoperative pain is essential in accelerating a patient’s recovery to normal function. It also minimizes adverse psychological effects that accompany acute and uncontrolled pain. Managing pain is instrumental in reducing the risk of developing complications after surgery, like pneumonia and blood clots. More than 75% of the patients who undergo surgery experience acute postoperative pain ranging from medium to high severity (Wang et al., 2019). When patients experience prolonged severe pain after surgery delays their discharge from a facility. While advancements have been made in pain management, not all clients experience complete relief from postoperative pain. The sharp increase in complex surgical procedures has necessitated perioperative and postoperative pain management.
The project focused on implementing the Enhanced Recovery After Surgery (ERAS) Program. It is meant to streamline the patient processes throughout the process. The program focuses on before, during, and after surgery (Macones et al., 2019). It shortens the length of the clients' stay and facilitates recovery while improving their experiences. The program aims to reduce postoperative opioid consumption. Consequently, the project reduces long-term opioid use, dependency, and addiction risks.
The project was implemented at Jamaica Hospital Medical Center, a private, non-profit teaching hospital and emergency facility located in the Jamaica neighborhood of Queens, New York City. The facility has a capacity of 404 beds and has served the Jamaican, Queens community since 1891. The hospital has a good reputation for providing patient-centered and expert care to the community around. I discovered the need for the project while working with pre-op and post-op patients. Their biggest issue was pain management mostly after surgery. Most of them expressed the challenge that after the surgery, they had instances of severe pain. Some patients feared more pain could develop after surgery and opted to stay in the facility longer.
Measuring outcomes
The outcome of the project will be measured in two ways. Firstly, the willingness of the multidisciplinary team to implement ERAS will be established. The project's success depends largely on how the interdisciplinary team consisting of surgeons, nurses, pharmacists, and dieticians will be willing to implement the protocol. The professionals' ability to follow the protocol phases will be measured. The project will be deemed successful if the compliance rate is high. The second approach would involve establishing its effectiveness in the patients. After implementing the project, a survey would be done to establish the severity of pain among patients after surgery. The project would be successful if the professionals managed to control pain.
The project is sustainable because it can be replicated. Once training is done on the care providers regarding ERAS, the same can be done to another batch. The training can be done depending on the availability of the healthcare workers. Moreover, the staff who have undergone the training can further train their colleagues. For instance, if nurse leader undergoes the training, they can organize training sessions for their team members. Hence, the project is sustainable in the long run.
Barriers
The barrier experienced was the unavailability of professionals for training on how to use ERAS. Healthcare professionals in the facility have tight schedules. Nurses, in particular, work under tight shifts. The facility does not have adequate healthcare professionals, meaning that the available ones have to work for longer hours. The apparent shortage made getting the professionals for a training session challenging. Most of them were held up while trying to attend to the patients. As a result, I had to undertake the training for a longer time than usual. Further, the tight schedule denied most staff an opportunity to particulate in the training session. This limited the number of healthcare workers who could undertake the training simultaneously.
