Introduction
This reflection contemplates implementing learning from an exclusive process of evidence-based practice in nursing for elderly patients' medication adherence. Fundamental knowledge and skills gained through this course directly address the significant problem of non-adherence to medication regimens in the older population. This project, which integrates theoretical learnings with practical methods (personalized medication management, supportive technologies), addresses the multiple barriers elderly patients face and retain in maintaining adherence to their prescribed medication. The role of the course in bridging the gap between academic knowledge and clinical application in such a practical manner is the course's essential contribution to addressing real-world healthcare challenges to improve patients' outcomes and quality of life for older adults.
Key Learnings from the Course
The course greatly enhanced my understanding of evidence-based practice, patient-centered care, and interprofessional collaboration needed to develop interventions for addressing medication adherence in elderly patients. The course integrated core concepts from multiple subfields of healthcare, emphasizing the crucial role of evidence in designing and identifying the most effective interventions and meeting the needs of the affected population (AACN, 2021). Further, the methodologies related to care appraise and synthesize research findings to inform clinical decisions critically related to multiple cases, which assisted me in evaluating the medical adherence issues among the elderly population above 50 years. Developing strategies to improve medication adherence, such as simplifying the medication regime and using technological aids like electronic health records and reminder systems to prevent non-adherence due to insensibleness or cognitive problems prevalent among the elderly, have been instrumental.
Learning from domain two, the other pivotal theme of the course was patient-centered care. The benefit of understanding the patient's context, including the patient's socio-economic background, health literacy and psychosocial factors, was stressed. Further, the course emphasizes the value of interprofessional collaboration in improving patient outcomes (AACN, 2021). Being exposed to this collaborative way of working will help me apply this to my ability to work in multi-disciplinary teams, deliver comprehensive care plans, and create solutions for medication adherence from different perspectives, including education and support services coupled with pharmacological management.
In addition, integrating nursing science with knowledge from other disciplines foreshadowed the implications of interdisciplinary knowledge for developing more robust interventions specific to the care of the elderly (AACN, 2021).
