The new healthcare system paradigm driven by the introduction of the Affordable Care Act (ACA) established nurses’ priorities. In addition, it exposed the disenfranchised nature of medical care in the United States (U.S.). Patient-Centered Medical Homes (PCMHs) is an innovative healthcare delivery framework that emerged with the advent of the ACA and is anchored on interdisciplinary delivery teams (Cleveland et al., 2019; Wasserman et al., 2019). PCMHs mainly deal with chronically ill patients and emphasize cost savings via care coordination and communication. This delivery model positively impacts patient outcomes by ensuring that care is provided across the health services continuum and that the medical homes are reimbursed based on the number of clients they serve (Cleveland et al., 2019). In this context, the delivery model ensures patient-centered care to enhance self-management capacity (Burton et al., 2017). This allows nurse practitioners and clinical nurses to use transformational leadership competencies to positively impact patient outcomes and the interdisciplinary team to create quality-driven, patient-centered, and innovative health management plans (Cleveland et al., 2019).
