The Impact of Aftercare through Online Platforms on Reduction of Patient Readmission Rates
In the United States, about half of Medicaid and Medicare beneficiaries nationally are readmitted within 30 days after they are discharged from hospitals (Jackson et al., 2015). According to Jackson et al. (2015), this problem, which faces most of the healthcare systems across the world, is attributed to lack or insufficient outpatient follow-up after discharge. In this respect, the proposed study will seek to investigate the effect of aftercare using online tools in reducing readmission rates Jackson et al. (2015) observed that outpatient follow-up is one of the key strategies used in the United States to reduce rehospitalisation rates. To avoid cost and resource implications in aftercare, the proposed study suggests the need to utilize online platforms to carry out follow-ups for patients discharged. The stakeholders expected to benefit from the findings of this study include the patients, the healthcare providers, and the government.
