The author of this article believes that by implementing an initiative to encourage patients to maintain comprehensive lists of their medications, a more collaborative approach could be achieved, therby improving the accuracy of medication reconciliation and facilitating communication between patients and healthcare providers. 2 Training and Use of Smartphone Applications Another effective process is the conduct of regular training sessions for healthcare providers on the importance of medication safety and the use of smartphone tools, alongside informational sessions for patients on maintaining and utilizing medication lists. 3 Medication Safety Committee Finally, the author of this proposal also believes that merely implementing this project without a more collaborative and holistic approach could waste its potential. Thus, a temporary medication safety committee should be created, which would oversee the implementation of the quality improvement plan, monitor progress, and address any challenges encountered. Impact on Healthcare Delivery System The complexity of healthcare delivery, especially during transitions from hospital to community settings, presents significant challenges, including the risk of medication errors and related harm.
