The purpose of the project
The main objective of implementing quality improvement initiative in clinical pathology is to ensure that there is zero-defects performance in the process of carrying out diagnostics on the patients. In this case, defects has been define to mean deficiencies in specimens, imperfections and flaw of procedures of collecting samples thus resulting to misdiagnosis, delays, stoppage or repeat of the process. Critical analysis on these defects showed that it has nothing to do with misinterpretation of laboratory results and non-diagnostic defects, but are as a result of quality issues (D'Angelo & Zarbo, 2007).
Recent audits conducted in pupils hospital showed that at the rate of 27.9 % as the baseline, 89.3 % of all the defects arises from laboratory procedures, 8.3 % came from pre-analytic stage and only 2.4 % was contributed by the final reporting. Further specific audit done in the laboratory procedure showed that 78 % of the defects are caused by slides and block. In order to figure the reality in the field, results were expounded to reflect the number of casesthat are likely to occur after million surgical pathologies has been performed, and the outcome were that approximately 4.3 to 4.8 sigma are likely to occur per million opportunities. These would require extra 159 hours for three week to correct the mistakes and deliver accurate results to the patients (Zarbo & D'Angelo, 2006).
Despite of clear benchmarks and standardize procedures of collecting laboratory samples as stipulated in Collage of American Pathologists Quality Assurance Committee manual. Technician may become complacent with shortcuts or operate mediocrely, thus compromising the quality and integrity of the results. Therefore, implementation of quality improvement initiative would strive towards achieving zero-defect performance, reducing waste and misidentification in clinical pathology.
The target population or audience
Implementation of quality improvement initiative targets all the health workers since they are affected in one way or another. But the most important group is the laboratory technicians who collect samples, prepare specimens, analyse the specimens and record the results. Also the results obtained would determine the appropriate type of medication that a patient should be administered to contain the illness or treat the disease. Perhaps, doctors, clinical officers, nurses, technicians and administrator contribute in one way or another to these defects since they occur at different stages of treatments. Although the data used to formulate the proposed quality improvement initiative program was collected from all stage of treatment, substantial part of the results relate to laboratory analysis, thus laboratory technicians are the most targeted groups.
The benefits of the program
Reduce waste: This program would greatly reduce the wastes that are experienced in many public hospitals. For instance, the time spends waiting for the results that are repeated or delayed affects the productivity of an individual worker per day. Also this would reduce the number of patients attended per day. Repeated laboratory analysis would involve the use of reagents and chemicals that would have used to serve another patient.
