Unit 8 Journal: Transgender Youth Case Analysis Purpose This journal entry is aimed at using evidence-based psychiatric assessment, diagnostic reasoning, and treatment planning on the case of a 14-year-old transgender male adolescent. This case is particularly delicate since it is one that concerns depression, suicidal ideation, gender dysphoria, a history of trauma, and family dynamics, which play a significant role in clinical decision-making and overall stability of the patient. This journal describes the use of correct screening tools, labs, differentiation diagnosis, working diagnosis, a comprehensive treatment plan, culture, and ethical issues. 1. Screening Tools and Laboratory Tests Screening Tools In order to understand the presentation by Joseph correctly, I would use: PHQ-A - to determine the severity of depressive symptoms. C-SSRS- necessary because of his suicidal ideation and intent. GAD-7 - to measure anxiety, which he evidently has difficulties with. UCLA PTSD Reaction Index - due to his traumatic experiences in the past. Vanderbilt ADHD or Conners-3 - to identify the causes of concentration problems, for example, ADHD or depression. GIDYQ-AA - to measure gender dysphoria and its effects. NSSI-AT - significant as a result of self-harm in the recent past. Laboratory Tests The following labs would also be ordered by me: CBC - eliminate anemia or infection. Vitamin D + B12 deficiencies can increase fatigue or mood. HbA1C- based on weight gain and appetite change. CMP - test electrolytes, liver, and kidney functions. TSH + Free T4 - detect depressive symptoms of thyroid. Urine Drug Screen- routine in psychiatric admissions. Lipid Panel- required in future gender-affirming care. Urine pregnancy test- needed prior to medication change. 2.
