Assignment Overview
Given that each health care system and facility faces unique site-specific risk challenges, a one-size-fits-all model does not exist in risk management. Health care managers must consider all aspects of their setting very carefully in terms of patient safety, mandatory regulations, potential medical errors in the setting, existing and future policies/procedures, as well as current and upcoming legislative acts that affect health care provision in the setting.
This module’s Case Assignment challenges you to locate and read an existing risk management and patient safety plan of a facility of your choice, and then assess its components in light of the background readings.
Case Assignment
Using the information in the required readings as well as some additional research in peer-reviewed sources, complete your Case Assignment by answering the following:
Choose a health care facility (hospital, physician’s office, freestanding ambulatory clinic, etc.), and summarize each category of its risk management and patient safety plan. This can be your current or future-intended setting, or a facility that you locate through some research online. What is defined/included in the components of the plan? Who are the stakeholders responsible for the implementation of each component?
Are the directives stated by the American Society for Health Care Risk Management followed in the plan? If not, what seems to be missing?
What could be added/clarified in the plan’s policies and/or procedures to either further prevent risk or enhance patient safety? Be as specific as you can in your assessment.
Does the facility have a dedicated Patient Safety Officer (PSO)? What is the role and scope of a PSO’s daily work?

The facility located via online research to analyze in this paper is The North Simcoe Muskoka Local Health Integration Network (SML LHIN). NSM LHIN is a crown agency that reports to the Ministry of Health and Long-Term Care in Canada, and it is one of the facilities I anticipate working with in the future. This essay delineates the risk management and patient safety plan at NSM LHIN, links the American Society for Health Care Risk Management (ASHRM) directives to the plan, and evaluates the role and scope of practice of a Patient Safety Officer at this facility.
Health Care Facility and Risk Management and Patient Safety Plan Summary
The risk management and safety plan for NSM LHIN has four elements: leadership and accountability, performance measurement resources and tools, quality, risk and safety education and awareness, and quality, risk and safety culture. In the leadership and accountability framework, NSM LHIN has guiding principles within the organization and with their vendors and service provider organizations. Healthcare management regards patient safety and quality as a strategic objective, and this is done by implementing an effective quality improvement plan, installing quality structures at different governance, leadership, and staff levels, and policies that reflect legislative and regulatory changes in quality improvement patient safety. Service providers and vendors for NSM LHIN are also tasked with collaborating with the administration to mitigate risks and ensure quality in all processes (North Simcoe Muskoka, 2019). The facility requires the contracts team to be thorough in their selection and organize quarterly or ad hoc meetings to address concerns, issues, and opportunities.
In the performance measurement, resources, and tools framework, the facility actively monitors and reports all risks in a Quality Improvement Plan (QIP).
