Coursework Instructions:
After reviewing the 2012 QSEN Graduate Competencies (PDF), identify a quality/safety issue that you have seen in your practice or clinical setting. Explain to your classmates the issue you have identified and the reasons why it is an issue. Include the potential negative outcomes that can arise if the issue is not resolved. For example, you notice that the provider is not following the American Cancer Society's recommendation for cervical cancer screening. This issue in quality competency can lead to under-testing, over-testing, increased cost, and increased potential for surgical procedures. These factors can have negative consequences, such as higher risk for preterm labor or infertility. As you create your post, review the knowledge, skills, and attitudes of the relevant competency regarding the quality/safety issue you have identified. I work in Psychiatric mental health settings but in school for women's health. Please include references that are 2-5 years old. I am attaching the 2012 QSEN graduate competencies to this.

The patients can be referred to social workers, psychiatric nurses, and, in some cases, obstetricians. This means that interdisciplinary coordination is needed to make sure that the contributions of every provider are in agreement and that the best patient safety and quality outcomes are reached (Khatri et al., 2023). However, some information may be left out or misunderstood in case handoff communication does not follow the standard procedures or otherwise is poor. In the treatment of women with complex needs, the lack of information about menstrual or hormonal drugs, a history of self-harm, and pregnancy can trigger the emergence of critical errors. To this end, Chaharsoughi et al. (2025) in their study highlight that nearly 80% of critical medical errors are attributable to poor communication during the patient handover. Therefore, inadequate handoff poses serious risks to patients’ well-be...
