Screening procedures are necessary medical tests to check health conditions before symptom outcomes. The processes help to identify problems earlier when it is easier to treat. As such, different screening tools are designed to determine the potential presence of a particular issue and are typically used as a preliminary step in evaluation for more comprehensive assessments if necessary. An example of a psychiatric screening tool to discuss is Insomnia Severity Index (ISI).
Insomnia Severity Index (ISI)
Initially referred to as the sleep impairment index, ISI is a 7-item tool measure generated based on issues of clinical relevance to people seeking intervention for a sleep disorder. The standard mode of instrument administration is a pen-paper or alternate forms that clinicians can avail during clinical interviews. In that way, semi-independent yet complementary information can be retrieved. ISI instructions can be considerably altered for different versions by clinicians to provide reliable details or detect cases of insomnia (Manzar et al., 2021). Although the condition is prevalently a complaint with numerous consequences on the quality of health or life, it remains undiagnosed and untreated in clinical settings.
Therefore, ISI is designed as a specific tool for detecting insomnia, containing seven-item questionnaires that ask respondents to rate the nature or signs of sleep problems. A Likert-type scale is used to rate the severity of the condition, and the questions asked are related to the subjective qualities of clients' sleep (Okajima et al., 2020). They include symptoms severity, satisfaction with sleep patterns, degree of insomnia interference, how respondents notice or feel insomnia is to others, and the level of distress caused. The ISI scale is validated on separate insomnia patient groups aged between 17 to 84 (Otte et al., 2019). The tool administration can take about five minutes minimum for completion, where a self-report brief measure is administered with paper and pen.
The Interpretation of Scoring for ISI Tool
Upon administration of ISI questionnaires to patient targets, respondents are required to rate each element using a scale. Responses may range from 0 to 4, whereby the higher value indicates more acute symptoms. The goal is to assess the nature, impacts and severity of insomnia with past month recalls. Dimensions evaluated include sleep maintenance, the severity of sleep onset, problems waking up early morning, dissatisfaction with sleep, sleep interfering with daytime functions, noticeable sleep issues by others, and distress resulting from the condition. Each item is rated with a Likert scale, whereby a 0 score is equated to no problem and 4 for the condition severity to yield a total outcome range from 0 to 28. The final score is interpreted as; lack of insomnia falling within the range of 0 to 7, subthreshold insomnia from 8 to 14, moderate from 15 to 21, and severe problem from 22 to 28 (Thakral et al., 2021). Some people may report daytime sleepiness and at least one sign of insomnia several times a week. In that case, they are defined as having a subthreshold of the disorder and not fulfilling insomnia diagnosis.
Treatment Plan for Patients with Positive Results
The ISI tool's positive results may indicate chronic insomnia in tested patients. As such, the main objectives of treatment would include improving sleep quality or duration and lowering daytime impairments. An intervention plan must include behavioral intervention, lifestyle modification, pharmacological approaches, patient education on the condition, additional tests, and clinical follow-ups.
Non- Pharmacological Interventions
Chronic insomnia is linked to cognitive difficulties, poor work performance, depression or anxiety, and other health conditions leading to declined quality of life. Nonpharmacologic therapies are integrated into the recovery process, including techniques like cognitive behavior, relaxation, and sleep hygiene. According to Jernelöv et al.
