Purpose of Study
The articulation development in children with cleft lip and palate has long been a field of clinical interest, as the articulation error rates, compensatory speech errors, and the delay in achieving age-appropriate phonological development are high. Following continued disparities in the provision of specialized speech therapy in most areas, Prathanee et al. conducted a study to develop, pilot, and assess the efficacy of the Application for Articulation Therapy-Thai (AAT-T). The purpose of this innovative mobile therapy platform was the creation of a particular program to target the Thai-speaking child with a cleft lip and palate who articulates poorly. Their aim of study was thus to establish whether AAT-T, as a digitally based intervention tool, had the potential to become a clinically effective means of enhancing articulation accuracy and aiding the results of speech therapy. The reason why the researchers attempted to fill the gap was a pressing need the increased demand in the availability of accessible, culturally and linguistically adequate interventions that can be used as an addition, or even a complement, to the traditional in-person therapy (Salari). Their purpose statement emphasized the importance of offering an evidence-based digital tool to fill the gap in the resources of treatment, especially among children residing in underserved or rural areas.
Rationale for the Study
The motivation behind the study was also informed by a number of significant theoretical and clinical suggestions (Prathanee). Previous studies have shown that early and regular articulation therapy is very important in a child with cleft cleft-related speech disorder, but due to the shortage of workforce, barriers to traveling, and inaccessibility of speech-language pathologists, many Thai children still lack adequate articulation therapy intensity. Arbitrarily, articulation treatment has always been provided as a face-to-face intervention, which in many cases may be hard to maintain throughout the long period of treatment. The authors have identified that there was already an emergence of mobile applications and digital therapy platforms in other languages and in clinical settings, with promise as cost-effective, user-friendly, and flexible therapy options. But there was no similar mobile tool to be used among Thai-speaking children, and especially one specialized in the particular articulation error patterns related to cleft lip and palate. The use of technologies in mobile learning, recording the audiovisual material and interactive feedback provided the possibility to create a modern tool, which would be offered as a part of home practice or therapy. These circumstances encouraged the authors to work on developing Application for Articulation Therapy-Thai as one of the ways to enhance therapy intensity, promote child engagement, and achieve better results by means of structured digital practice.
The other important element of the rationale was that the recognition of the need to practice early intervention using a family-centered and home-based practice was growing within the speech-language pathology community. The studies revealed that the effects of family participation and the recurrent probability of generating target sounds greatly enhance the acquisition of articulation. The Application for Articulation Therapy-Thai application was therefore developed to give the parents and children convenient activities that could be practiced at home either by themselves or with supervision. This new literature on the effectiveness of digital therapy and clinical conditions encountered by Thai families provided a solid basis for the research of the authors. Their work was aimed at both establishing the clinical efficiency of the app and making a new contribution to the literature of digital articulation therapy of special populations.
