As a nurse with Doctor of Nursing Practice, education plays a major role in all the operations and the interventions that are incorporated in the practices. This is relative to the fact that, the professional background of a DNP leader is also based on education, with close reference to evidence based practices, improvements on the quality of care and boosting the leadership systems (Gallagher-Ford, Fineout-Overholt, Melnyk, & Stillwell, 2011). On a daily basis a leader in nursing is faced with quite a number of decisions that they have to make with reference to the profession, patients, coworkers and even the community among others. A DNP leader can concentrate on a number of aspects to make sure that they are streamlined, relative to the emergence of new information and therefore new practices that are based on improved theories. All of which have to be implemented in actions. To better elaborate on the best practices that a DNP leader can implement, this paper evaluates how one can bring about innovative cultural change in support of evidence based practices at the hospital. Evidence based practices in nursing are gaining popularity, especially considering the fact that they lead to better patient outcomes. One of the health problems that needs to be addressed using this approach is related to pica self-injurious behavior ("Mental Health and Pica", 2016). Children that develop this behavior are likely to cause injury to their digestive systems, furthermore the objects that are ingested do not have any nutritional value. It is common for children to try and get objects into their mouths; however, there is a limit to which this habit develops into a health problem as the children become obsessive in their attempts to swallow objects that they come across. Using education and evidence based nursing practices, it is possible to improve the level of care that is extended to such patients. This will better offer quality health care outcomes, compared to the traditional and standard approach.
Pica behavior in children
It is crucial to understand the basics of the Pica condition that can be quite dangerous if left unattended. Pica is an eating disorder that shows some aspect of persistence in trying to ingest objects that do not have any nutritive substance for at least one month at an age when this behavior would be considered to be developmentally inappropriate. While in some of the cases, it may be considered to be benign, in some of the cases, it is likely to have some fatal consequences if it is not treated. The clinical presentations of the condition are highly varied and they tend to be associated with specific nature of the medical condition that results as well as the object that has been ingested by the patient. It is the variability that comes with the objects that has been ingested that creates some level of difficulty when dealing with such cases ("Mental Health and Pica", 2016). There are some physical manifestations that may give away the main cause of the complication such as manifestations of the toxins ingested, manifestation of parasitic or infectious that the patient is showing, GI manifestations or even the dental manifestations where the child will show signs of surface teeth loss or tooth abrasion that can be considered to be severe (Ellis, 2016). Some of the complications that have to be addressed include nutritional effects of the behavior, GI complications and those that are associated with toxic substances such as lead which causes health complications in the renal system, cardiovascular implications, hematologic and neurological complications. It is crucial to note that diagnosis of the behavior is not as straight forward, relative to the fact that there are countless objects that a child can ingest relative to their environment. However, with reference to the gastrointestinal infections, there are a number of imaging studies that can be used in treating the patient, such as; Upper GI endoscopy, Abdominal Radiography along with upper and lower examinations of the GI using Barium. To better manage the complications that come with ingesting foreign objects, there is a need to use a multidisciplinary approach, to help develop a comprehensive health care plan that addresses all the challenges and the various information sets that can better improve the healthcare outcome. With more professionals on the team, it is much easier for the health care providers to brainstorm on the best practices relative to the fact that, complications are not associated with just one area of the health system (Ellis, 2016). It is also important to note that every other case is unique and will require a team effort to determine the best treatment plan, catering for all the health condition that may arise ("Mental Health and Pica", 2016). The complexity of the behavior can be largely associated with a number of factors such as developmental disabilities, mental retardation and/or obsessive compulsive disorder.The team would have to observe the patient for at least a month, after which they can come up with a winning treatment plan that will address all of these challenges.
Proposed project and planned intervention
The elemental problem that is associated with pica is that, it can be quite fatal if it is not addressed comprehensively. Most of the patients that have had complications, have been with parents or guardians that did not take precaution when they found out that the child is exhibiting some rather peculiar tendencies associated with odd eating habits ("Mental Health and Pica", 2016). It is until a child has developed some complications that the parents then take note. In most of the cases, these are emergencies as the child’s immune system is not quite well developed to handle some of the challenges such as lead toxicity in the body, which can lead to a number of very fatal and long-term conditions in the body (Jacobs, Jones, Gabella, Spring, &Brownson, 2012). This is relative to the fact that the child is still developing and any form of health complications that affects the major systems such as the nervous system or the renal system, as such, given the complications that can result from a child ingesting a foreign body that can cause both physical, chemical and even biological harm, there is need for s comprehensive approach that will address the challenges with such emergences in time to avoid permanent damage or fatalities.
Using education as a background for evidence based care is at the pivot of the intervention proposed in this paper. However, there is a number of other evidence based practices that are going to be incorporated to ensure smoothness. The planned intervention is going to be based on open communication channels. This will enhance the ground work for the cultural intervention that will better help the teams to exchange information in a timely manner and with high level of accuracy. As an effective communicator and DNP leader, it is crucial to consider that all the team members are given equal chances to interact with one another and the information available. Different professionals on the team will have different sets of information that is based on updated educational material. All of these information sets will then have to be put into action (Jacobs, Jones, Gabella, Spring, &Brownson, 2012). As such, having an open communication platform better enhances the speed with which information is availed, debated and formulated into actionable processes. Using the transformative leader approach, it is crucial that all the members of the team feel that they are valued and that their contributions will enhance the outcomes of the health care plan (Spahr, 2016). This means, that as a leader it is crucial that the rest of the team members regardless of their profession and expertise, are empowered to give their best in the evidence based practices. The open communication platform will better enhance their ability to take part actively in the treatment plan development and implementations, with regard to the different patients. This means mentoring the nurses to apply their skills and further enhance their skills through active education platforms, which will be crucial in the decision making processes. There is also the element of integrity that will form the backbone of the intervention, where the team of professionals and the parents are able to interact with absolute openness. This is relative to the fact that the intervention will also include a patient centered component. This means that the parents will also have to bring on their input (Jacobs, Jones, Gabella, Spring, &Brownson, 2012).
