Purpose

This assignment increases the students' awareness of the National Patient Safety Goals developed by The Joint Commission. Specifically, this assignment will introduce the Speak Up Initiatives, an award-winning patient safety program designed to help patients promote their own safety by proactively taking charge of their healthcare.

Course outcomes: This assignment enables the student to meet the following course outcomes:

CO #2: Apply the concepts of health promotion and illness prevention in the laboratory setting. (PO #2)

CO #8: Explain the rationale for selected nursing interventions based upon current nursing literature. (PO #8)

The brochure title is “Medication Mistakes.” It does not have an indicated date of publication. The brochure’s message targets patients at home or in healthcare facilities.
Summary of Brochure
The brochure indicates that mistakes happen daily. As a result, it provides questions and answers to prevent errors with a patient’s medicine. Many people are responsible for a patient’s medication, like doctors, pharmacists, nurses and other caregivers (The Joint Commission, n.d.). All these professionals work together to ensure that patients receive the correct medicine and dosage. When patients receive a prescription, they should ensure they can read the handwriting. Patients are free to ask for the prescription to be printed. The brochure indicates the need to read the label and ensure the correct name is on it (The Joint Commission, n.d.). In the hospital setting, patients should ensure that doctors, nurses, and other caregivers check their wristbands for their names before giving any medication.
Patient Communication
The brochure indicates that patients should be free to ask questions regarding medicines. For instance, patients are supposed to ensure they can read the handwriting on the prescription. They can also request to have it printed. Patients who do not understand the prescription can ask the doctor, pharmacist, nurse or caregiver for more details. The brochure advocates for communication where a patient cannot recall the instructions. In such a case, a patient should be free to call the healthcare professional and ask more questions about the medicines. In the case where a patient feels that a specific medication is not working well, there is a need to engage the healthcare providers. Patients should not take medication if they feel it is the wrong one. When they suspect they are about to be given the wrong dosage, they should speak up and inform the professional about the issue.
Healthcare providers are encouraged to ask patients their names before administering medication. In the healthcare setting, nurses and other caregivers attend to many patients. It is, therefore, possible to confuse a patient's prescription and give it to another. Hence, nurses must communicate with patients to ensure they administer the medicine to the correct patient. Healthcare providers need to address the concerns of patients. The brochure advocates for doctors, pharmacists, nurses and other caregivers to allow patients to ask questions.
