Introduction
Ventricular Assist Devices (VADs) are modalities that support the functioning of the heart ventricles and can be of different types. The right VAD, an external modality, supports the functioning of the right ventricle of the heart. An implanted left VAD is an equipment for pumping blood from the heart's left ventricle to other parts of the body, while a Biventricular device acts on both chambers (Khasbage et al., 2022). The chosen therapeutic modality is an advanced tool for the treatment of chronic cardiac conditions, especially for patients with heart failure at the end stage (Reid et al., 2021). It bridges the gap between treatment options such as transplantation therapy, is a bridge to recuperation, and serves as a destination treatment (Blanchet, 2024). For instance, an imaginary case that could be used to show how VADs are applied in heart failure treatment may involve a 65-year-old male patient (H.M) who has been diagnosed with left ventricular heart failure and has missed heart transplantation schedules in the hospital he is being attended to, due to lack of a match for the procedure. This condition has deteriorated; therefore, he fits the indications for a Left VAD to help his heart function.
Explanation and Background
Shortages in donor hearts have prompted researchers to develop VADs, mechanical pumps to take over the work of the heart partially or entirely. They help the ventricles pump blood and thus relieve the workload on the weakened heart for patients with chronic heart failure (Zaidi, 2021). LVADs offload the left ventricle, while RVADs offload the right ventricle of the heart. These devices consist of three parts: an implantable device placed inside the body or an external paracorporeal part, a system controller of the VAD settings, and a power source for the pump (Andrade et al., 2021). For the RVAD, blood moves in a cycle out of the right ventricle, enters the pump, and then goes to the pulmonary artery and lungs for oxygenation. For patients using LVADs, blood exits the left ventricle into the pump, then into the aorta, and to all body parts. Oxygenated blood is moved from the left atrium or ascending aorta to the rest of the body. The modality is attached to an external console plugged into an outlet in the wall or connected to a battery pack worn over the shoulder (Andrade et al., 2021). The most common population for which these devices are used is those with severe heart failure who have not been responsive to medical or surgical treatments. The main factors considered in their application include the likelihood of infections, indication for anticoagulation treatment and the cost implications to the individual and the healthcare system.
Risks and Benefits
Implantation of LVADs increases strength and improves the patient's quality of life, with more guaranteed functional support and survival (Di Nora et al., 2021). These positive outcomes are realized upon careful selection of patients, a focused surgical installation, and thorough postoperative patient care that entails mental and physical rehabilitation. However, some patients experience complications such as bleeding, infections at the incision or in the heart, stroke, hemolysis, dysfunction of the right heart side and even death (Vaidya et al., 2020). These risks call for more informed care of patients involving frequent monitoring for infection signs, medication adherence, especially anticoagulant therapies, and educating the patients on how to use the device to avoid its failure effectively.
