Developing an Implementation Plan (NRS-441V)
Question Consider the population in which the solution is intended, the staff that will participate, and the key contributors that must provide approval and/or support for your project to be implemented. These stakeholders are considered your audience. Develop an implementation plan (1,925 words). The elements that should be included in your plan are listed below: Method of obtaining necessary approval(s) and securing support from your organization\\\'s leadership and fellow staff. Description of current problem, issue, or deficit requiring a change. Hint: If you are proposing a change in current policy, process, or procedure(s) when delivering patient care, describe first the current policy, process, or procedure as a baseline for comparison. Detailed explanation of proposed solution (new policy, process, procedure, or education to address the problem/deficit). Rationale for selecting proposed solution. Evidence from your review of literature in Topic 2 to support your proposed solution and reason for change. Description of implementation logistics (When and how will the change be integrated into the current organizational structure, culture, and workflow? Who will be responsible for initiating the change, educating staff, and overseeing the implementation process?) Resources required for implementation: Staff; Educational Materials (pamphlets, handouts, posters, and PowerPoint presentations); Assessment Tools (questionnaires, surveys, pre- and post-tests to assess knowledge of participants at baseline and after intervention); Technology (technology or software needs); Funds (cost of educating staff, printing or producing educational materials, gathering and analyzing data before, during, and following implementation), and staff to initiate, oversee, and evaluate change. Developing an Implementation Plan
In this paper, an implementation plan is developed. This implementation plan covers the method of obtaining the necessary approvals and securing support from the leadership and fellow staff members of my organization. The current problem is described in detail. This paper also provides detailed explanation of the proposed solution in order to address the problem, along with a rationale for selecting the proposed solution. Evidence from the literature review is provided to support the proposed solution. The implementation logistics is provided, including how and when the change would be included in the current organizational structure, culture, as well as workflow; and people who would be responsible for initiating the change, educating employees, and overseeing the implementation process. The resources needed for the implementation are described, along with the funds that would be utilized in the process.
Method of obtaining necessary approvals and support
Approvals and support from my fellow workers and the management would be obtained primarily through consulting and involving them in the change process. This will help to reduce resistance and obstruction of the change. Face-to-face consultative communications will be done to get approval and support for the project, and this communication will be done regularly. It is of note that involving and notifying people will basically create opportunities for other people to partake in both planning and executing the changes; this will serve to ease the burden, spread the organizational load, and also create a sense of familiarity and ownership amongst those affected (Pexton, 2009).
For this particular project, the key stakeholders include adults with hypertension; the staff in the primary care setting that will take part; as well as key contributors – the top leadership of my organization and fellow workers – who have to provide approval and/or support for my project to be implemented. To obtain the necessary approvals and securing support from fellow staff and the organization’s leadership, communicating to them the rationale behind the need for change will be imperative. In essence, I will explain to them the reasons why it is vital and necessary for the change to take place, and the projected benefits associated with the change. It is noteworthy that this will have to be handled in a careful manner and be communicated to every party that is concerned and/or affected (Pexton, 2009). Each of the affected/concerned parties would be given enough opportunity for raising their worries and/or contribute their opinions, viewpoints, and feelings. With the use of this method, it would be possible to convince the leadership of the healthcare organization and my colleagues to approve and support my project. Once they understand the expected benefits of recommending exercise regimens and physical activities for hypertensive adult patients with the objective of lowering their blood pressure, then they will most likely approve and support the project.
Description of the current problem
The identified problem is hypertension in adults – age 18 years and above. Hypertension (HTN) is essentially a persistent medical condition wherein the blood pressure within the arteries is raised. HTN is generally defined as HBP readings which are above, or at 140 mm Hg for the upper number or at least 90 for the number at the bottom. Roughly 67 million persons or 1 in three American adults have HBP (Centers for Disease Control, 2014). Pretty much 47 percent of individuals with HTN have their condition under control. HBP, a key risk factor for stroke and heart attack, affects over 26% of the global population, and contributes to over 13% of premature mortalities (American Heart Association, 2013). HBP is known to raise an individual’s risk for getting stroke or developing heart disease; the 2 chief causes of death for American adults. This problem is observed within the primary care settings.
At the moment, medicines for HTN have lots of side effects that patients frequently raise complains about. Diuretics are known to cause erection problems in some men; extra urination; abrupt and severe foot pain; as well as fatigue and leg cramps. Beta-blockers also cause erection problems, besides causing asthma symptoms; sleep problems and insomnia; depression; and cold feet and cold hands. Bet-blockers essentially make the heart of the patient to beat more slowly and less forcefully. ACE Inhibitors result in loss of taste; skin rash; and a hacking, persistent dry cough. These medicines hinder the formation of a hormone which makes the blood vessels to become narrow (WebMD, 2014).
Description of proposed solution and rationale
To address the problem, the proposed solution is to do regular exercises in order to reduce HBP. Having HBP and not getting adequate exercise are closely related. For individuals with HBP, exercising regularly can help them in controlling the medical condition. According to the American Heart Association (2013), doing exercise or engaging in physical activities is essential for blood circulation and for the heart. Regular exercise, for instance walking, has been demonstrated to reduce HBP. Exercising regularly will make the heart to become stronger. It is of note that a heart which is stronger could pump more blood using less amount of effort. If the heart could work less in pumping blood, then the force on the arteries will decrease, thereby lowering the blood pressure (Jorge & Paula, 2012).
Evidence from literature to support proposed solution
A number of research studies done by several researchers have consistently demonstrated that doing exercises and engaging in physical activities will reduce blood pressure. From their research findings, Cornelissen and Smart (2013) established that isometric, dynamic resistance, as well as endurance training reduce systolic blood pressure (SBP) and diastolic blood pressure (DBP). Combined training reduces only diastolic blood pressure. As such, doing exercise helps to reduce HBP and this will lessen the side effects linked to the use of HTN medication.
Dimeo et al., (2012) conducted a research study and their findings revealed that physical activity to a great extent lowered both diastolic and systolic daytime ambulatory BP by 6 ± 12 mm Hg and 3 ± 7. Exercising regularly actually lowered BP on exertion and increased physical performance as evaluated by lactate curves and maximum oxygen uptake. Doing exercises can lower BP even in individuals who have low responsiveness to medical treatment (Dimeo et al., 2012). Fox (2012) pointed out that exercising regularly reduces BP through improving the elasticity of the blood vessels, offering more flexibility, as well as giving less resistance to the f...
