Abstract
Manassas City is one of the cities in Virginia commonwealth with a total population of about 37,821. This research paper will involve a discussion on community nursing diagnosis and related nursing interventions. Besides, it will explore community nursing diagnosis for Manassas City and its readiness for self-health management concerning community health. Community health nursing is one of the standard practices that help advance and safeguard community members’ well-being. In such a case, a nurse acts as a case manager, educator, consultant, and healthcare provider in collaboration with community members to enhance the well-being of community members. The research paper will help to reveal nursing interventions to educate the community members about positive health choices, such as diet and regular exercise.

Manassas City is one of the independent jurisdictions located in Northern Virginia. Health is one of the major concerns for the community members, which influences the government to invest heavily in the sector. Nurses have a core obligation to educate community members on ways of enhancing their well-being through effective interventions. The city has significant health disparities among people of different ethnicities, incomes, and races. Although it has enough resources, some of the residents encounter challenges in accessing healthcare services. One of the contributing factors to the variations is the lack of equal distribution of resources, especially in underserved areas (Walker, 2018). Besides, the cost is a significant barrier limiting access to healthcare services, especially among uninsured residents. Manassas City has a high rate of uninsured individuals who encounter challenges in accessing timely healthcare services. For this reason, the department of community development works collaboratively with policymakers to accomplish the core objective of maintaining a high quality of life for the residents.
Community Nursing Diagnosis
Community nursing diagnosis involves a report by nurses that provides the focus of nursing care to a patient. The diagnosis can reveal the state of health that can direct care planning that falls under the nurse’s practice decision. In most instances, a “patient” can be a community, family, or individual. According to McDonell (2018), a registered nurse utilizes nursing diagnosis to act as a diagnostician. However, such a process involves making relevant decisions based on gathered data. Notably, a diagnosis is critical that includes an individualized approach to a patient’s situation, personal preferences, and needs by serving the components of the nursing process. Therefore, community nursing diagnosis is a valuable tool within a nursing profession based on an integration of healthcare practices to enhance the well-being of community members.
Maternal complications, Covid-19, cancer, HIV/AIDS, injury/violence, diabetes, and heart diseases are the common health issues across the Manassas community. According to statistics, the prevalence of HIV/AIDS is highest among adults aged about 50 years than in younger adults. The leading cause of the variations could be that seniors are unlikely to use protection and their immune systems are weak. Besides, older adults are likely to suffer from injury and violence, resulting in increased hospital admissions and fall rates. Similarly, lack of enough knowledge is a leading cause of maternal complications that result in high maternal deaths across Manassas City. Therefore, nurses ought to realize the need to create awareness on the need to be active to prevent and delay illnesses.
Manassas community encounters high rates of infant and maternal complications as a prevailing issue among the populace. However, nurses are committed to educating the community members on maternal issues to reduce the overall rate of maternal-related deaths (Melo & Alves, 2019). In such a case, the community members can adopt effective practices, such as immunization, towards improving the health levels of both the mother and infants. Besides, nurses have adopted home-visiting programs to assess participants' overall health and provide health services as needed. The core objective of the mechanisms is to improve the well-being of both the mother and infants. Nurses work collaboratively with community workers in delivering the needed health services.
Nursing Interventions
Active listening to patients is one of the nursing interventions that can help address the dominant health issues in the community. The strategy is a task that nurses complete with every patient that they come across. Although active listening is part of behavioral health, it is an effective way of supporting patient’s wellbeing (Perry et al., 2019). Secondly, preventing falls is an equally effective nursing intervention, especially in dealing with patients with diabetes. Nurses understand the risk of falling and injuring themselves among the patients. Accordingly, it is imperative to educate patients on the need to use bed alarms while using non-skid socks.
