Identify, or predict, the potential barriers to change. Discuss possible ways to overcome these obstacles, including methods for dealing with emerging or unforeseen circumstances that could impede implementation.
Describe the evaluation methods you will use to determine the level of success of your change initiative. Discuss what metrics or measureable determinates you will use.
Propose strategies to anchor change or support continuous change.
Establish how your change plan supports the organizational mission/goal, genuinely addresses stakeholder concerns, and will serve as an equitable contribution for the community or society overall.

The proposed change initiative is implementing purposeful (intentional) hourly rounding, which improves patient outcome, safety, and quality of care. There is a need to prevent falls and closely monitor patients, but nurses are also busy and attend to different patients. The institution has patient fall prevention protocols. As part of providing high-quality care, there is an emphasis on nursing lead initiatives supported by the best evidence to support decisions. Knowledge on implementing purposeful hourly rounding will allow nurses to focus more on patient safety and outcomes. Improving the quality of care and fall-related outcomes are useful indicators of the intervention's effectiveness.

The main internal force is patient fall prevention, as a reduction in the fall rate is one indication of improved health delivery. Healthcare practitioners are expected to provide patient-centered and safe care while reducing the rate of patient falls will also help reduce medical costs, and fewer patients will stay in the hospital. Closely monitoring the patient is recommended as a way to address their immediate needs and support personalized health care planning (Sims et al., 2018). Nurses and patients are the most impacted as it is nurses who implement purposeful hourly rounding and monitor patient progress. Nurses also prioritize the patients' needs in their interventions to improve the quality of care. Patient safety is a top priority in health institutions. It is more likely that patient satisfaction will improve as there will be a lower risk of patient falls and injuries (Spano-Szekely et al., 2019). The nurses closely monitor patient needs and make changes to ensure safe practice.
Externally, hospitals are expected to demonstrate they provide quality care in a healthcare system that is more efficient. Among the performance indicators in hospitals are patient fall rate and hospitalization stay. Stakeholders expected high-quality health service provision and appropriate care to promote healthy communities.

Among the stakeholders involved in the change, the process is the nurse, nurse leaders, and the patients. Nurses are involved in implementing the change initiative and providing quality healthcare. They closely monitor patients and undertake purposeful hourly nursing. There are changes in practice so that there are no longer irregular visits, and instead, there is purposeful rounding (Jenko et al., 2019). Patients are important as the intervention affects the quality of health and health outcomes of the standard treatment given to patients with this condition. Nurse leaders promote and support change initiatives. Their support is crucial to the success of the intervention as there is planning and setting aside resources to ensure the intervention is successful. Other healthcare workers support implementing the interventions and ensuring there will be an efficient workflow.

Promoting change in the organization requires willingness and the ability to change, and as an agent of change, I will facilitate the change process. Developing actions and promoting change with proactive improvements to help transform the organization and ensure improvement and effectiveness. There would also be a focus on collaborating with other stakeholders to promote change and support the change process. There will also be a change in behavior and attitudes towards change.
The transformational leadership theory is a model where the leader works with teams to identify the necessary change, creating a vision to guide the change through inspiration. There is a focus on executing change together with committed team members. The leader's behavior influences and inspires followers to work beyond their perceived capabilities and inspires them to achieve exceptional results. There is autonomy in making decisions where there is a positive change for the followers, and the leadership model is associated with increasing follower engagement, loyalty. There is an emphasis on adapting to different situations.

The change agents include trainers, nurse leaders, and other health care personnel. The trainers promote patient education and empower the health practitioners and patients. The nurse leaders direct and guide change, and these leaders are visionary. The leaders work closely with different healthcare workers and ensure the change project is successful with a focus on improving patient care. The other health care personnel facilitate and help in implementing changes in the workplace, and with the support of nurse leaders, there is support for clinical changes and improving the quality of patient care. The other health care practitioners, including other nurses, facilitate change and impact change.
