All parts of each question are to be answered.   Information / Materials provided: This is an open book assessment.   Assessment Criteria: To achieve a satisfactory result, your assessor will be looking for your ability to demonstrate the following key skills/tasks/knowledge as outlined in the marking criteria for this assessment task.    Number of Attempts: You will receive up to two (2) attempts at this assessment task. Should your 1st attempt be unsatisfactory (U), your teacher will provide feedback and discuss the relevant sections / questions with you and will arrange a due date for the submission of your 2nd attempt. If your 2nd submission is unsatisfactory (U), or you fail to submit a 2nd attempt, you will receive an overall unsatisfactory result for this assessment task. You must complete this assessment task by the due date provided or you may receive an unsatisfactory (U) result. If you are unable to meet a scheduled assessment due date, you must notify your teacher at least 48 hours prior to the due date to request an extension.   Case Study 1 Mrs Edith Jackson is an 82-year-old lady who was recently admitted to The Golden Days Nursing Home where you work. Edith has Parkinson’s disease and early stage Parkinson’s dementia. She also has low vision and is hard of hearing and wears glasses and hearing aids in both ears. Michael is Edith’s son and her sole support person.   a)             How would you make Mrs Jackson feel comfortable, using appropriate non-verbal communication (discuss at least three (3) points)?        Non-verbal communication involves the use of gestures, variation of pitch and tone of the voice, facial expressions, eye contact, and physical distance (proxemics) to convey the intended message. It strengthens nurse-patient relationship, and I would use it to make Mrs Jackson comfortable by;
  • Ø Using the proper gestures when talking to her to ensure my message is received effectively.
  • Ø Giving her personal space when seated next to her to respond to her questions. I would allow her to finish talking before I respond (Tiwari, 2015).
  • Ø Using flash cards and body language, making eye contact, and using therapeutic touch to make her comfortable and relaxed and ensure she understand the message better.
b)             How would you make Mrs Jackson feel comfortable, using appropriate verbal communication (discuss at least three (3) points)? Verbal communication involves using words to transmit the intended message. I would use it to make Mrs Jackson comfortable through;
  • Ø Introducing myself calmly and assuring her that I would be of assistance to her.
  • Ø Using simple language and being polite to ensure we communicate effectively.
  • Ø Asking her questions in a systematic manner by allowing her to respond before I ask the next one. I would also listen to her carefully and assure her of confidentiality of the information she shares (Colombo & Rampino, 2016).
  • Ø I would request to take her around for a walk so that I get an opportunity to see the challenges she is facing when walking. I would do the walking slowly and ask her to tell me when she feels difficulty in walking.
c)             Referring to the Core standards for nurses who provide support to people with disability, outline three (3) strategies that you could use to aid communication with Mrs Jackson giving consideration to her vision and hearing impairments.
  • Oral/Aural communication; I would introduce myself once I enter the room so that Mrs Jackson can see my lips and face when speaking to her. I would speak concisely and clearly.
  • Alternative and Augmentative Communication (AAC); I would use gestures, facial expressions, and body language (Frankel & PGCMS, 2019). I would also ensure that such aided devices as digital devices and written messages of shapes, pictures, and letters are touched and converted to speech for her sake.
  • Sensory integration therapy; I would seek her consent to take her arm and guide her as we walk, thus earning her trust and making her comfortable. I would also ensure background noise is limited and adequate lighting is provided.
You are walking past Mrs Jackson’s room and discover that she has fallen out of the bed onto the floor where is crying quietly and trying to get up. You assist Mrs Jackson onto a chair as she keeps saying over and over, “I’m alright”. After following facility procedure for this incident, you are then to discuss this with the patient and the son.   d)             Provide a definition of ‘open disclosure’ and discuss the principles of open disclosure referring to the Australian Open Disclosure Framework (2013) (min 100 words). Open disclosure is an open discussion with the patient on the incident that resulted in her harm or injury, as she receives medical attention. The discussion could also involve her support, carers, or family members. The principles of open disclosure include the following;
  • Ø Open, timely communication; the patient should give the patient accurate information on what happened in an open, timely, honest manner.
  • Ø Acknowledgement; the patient and family should know the adverse effects as soon as possible (Lock, 2019).
  • Ø Expression of interest or apology; the patient or their family should receive apology for any adverse effect on the patient.
  • Ø Supporting and meeting the expectations and needs of the patient and their family.
  • Ø Integrating the appropriate risk management and improving the systems.
  • Ø Good governance of the facilities and patient health and safety.
  • Ø Ensuring confidentiality of patient data and information.
e)             Provide an example conversation of what you would say when communicating the incident to the patient and son (min 100 words). Hello and welcome to this short briefing. My name is xxx and I am in charge of Mrs Jackson. I am here to brief the patient and you on the incident that took place this morning. Today, Mrs Jackson happened to fall from her bed, but she was attended to immediately and accorded the appropriate assistance and attention. A doctor was also called to attend to her and assess the extent of her injury. The assessment showed that despite the unfortunate incident, Mrs Jackson did not sustain any injury. Therefore, I am here to assure you that she is in good health and that she did not get any injury. We pass our apologies as this is not our intent. We are committed to protecting patient safety and we have already done the appropriate incident report on the same. Mrs Jackson is under close monitoring and evaluations for any change in her condition or any worse signs. We are ensuring she is comfortable and that she gets assistance whenever necessary. Thank you and I would be glad to respond to your questions. f)              Identify the person/s you would contact in this situation and provide a rationale. (min 50 words) This incident should be reported to the Register Nurse since she is the one working and collaborating with the RN to ensure patient safety. Further, several professionals would be required in this case; the physiotherapist would help in assessing Mrs Jackson’s situation, the medical team and social teams would also assist the patient. It is also the role of the nurse to handover the situation to the next staff on duty so as to be aware of it and monitor it closely.   g)             Using SBAR provide an example of what you would document in the patient chart ensuring that you use appropriate terminology. (min 50 words) SBAR is an effective tool for improving the efficiency of communication between healthcare practitioners (Toney-Butler & Unison-Pace, 2019). It would be used to document the following; Situation: Mrs Jackson fell from her bed in the morning. She started crying as she tried to get up. Background: Mrs Jackson is suffering the early stages of Parkinson Disease (dementia). Her vision is low and she is struggling to hear. She uses hearing aids in both ears and puts on glasses to improve her vision. Assessment: able to get up when assisted, she also affirms she is alright, and has stable vitals. Recommendation:  close monitoring is required. Medical review is also necessary as the fall could have resulted in injuries. Regular checks and close monitoring is also necessary. h)             Michael, Edith’s son will be traveling overseas for the next few weeks and will not have telephone access. He has asked if you could email him to provide him with a progress report on his mother. Identify at least five (5) factors relating to email etiquette you will need to adhere to when emailing Michael. (min 100 words) The email written to Michael should consider the following factors;
  • Ø The subject line should contain the content and salutation to the recipient.
  • Ø The information sent should be correct and confidentiality should be observed. It should be done in an honest and open manner. A brief description of the incident should be done to Michael, and briefing on the situation of the patient.
  • Ø Virus protection should be used to avoid corruption of the data or files attached.
  • Ø All lowercase or all uppercase letters should not be used. Using uppercase letters only implies that one is shouting. Professional courtesy should be observed.
  • Ø The message should not be sent to the junk emails.
  Case Study 2 You are working as an Enrolled Nurse in a rehabilitation unit and while on your break you check your social media account and notice that one of your colleagues, who is a friend of yours, has posted some information relating to one of the patients on the unit.   a)             What ethical principles and regulatory responsibilities do you need to consider in this situation? (min 100 words)        Social media practice should adhere to rehabilitation guidance and organizational policies regarding privacy and confidentiality. It is the role of healthcare professionals to ensure patient safety and confidentiality is protected (Smith & Watson, 2016). Patient confidentiality is mandatory and breach of the same could result in harm and significant adverse consequences such as legal pursuits.
