Change Topic (2-3 sentences): Bedside shift reporting is becoming a standard procedure in terms of handoffs for incoming and offcoming nurses. Because of its positive effects in nursing performance and efficacy, as well as patient outcomes, more and more institutions are implementing bedside shift reporting and replacing other traditional handoff procedures. Proposing standarized guidelines for bedside shift reporting can help with ensuring that the process is carried out completely and thoroughly to avoid risks of errors.

Article Title and Year Published

The secrets to successful nurse bedside shift report implementation and sustainability. (2018)

Moving Shift Report to the Bedside: An Evidence-Based Quality Improvement Project (2018)

Bedside Reporting is a Key to Communication  (2018)

Evaluation of bedside shift report: A research and evidence-based practice initiative (2018)

Research Questions (Qualitative)/Hypothesis (Quantitative)

Bedside shift report implementation and strategies for a successful practice

How does the implementation of bedside shift reporting in acute care impact patient safety, patient satisfaction, and nurse satisfaction?

Does BSR count as a good communication tool to improve patient safety, patient satisfaction, and nurse satisfaction?

What are the possible impacts of applying EBP in implementing bedside shift reporting to clinical practices?

Purposes/Aim of Study

To identify what strategies are effective in implementing bedside shift reporting in real-world settings

To determine if there are improvements in patient safety and satisfaction, and nurse satisfaction after the successful implementation of BSR

To assess how effective communication efforts are in BSR and identify gaps in implementation.

To explore perspectives in using BSR implementation and its outcomes in assessing the role of EBP in the process

Design (Type of Quantitative, or Type of Qualitative)

Qualitative, Literature Review

Quantitative, Statistical analusis for comparing before and after scores of the variables

Qualitative, Descriptive Analysis

Quantitative-Qualitative (mixed methods design)

Setting/Sample

English-language journals published from 2006 to 2016 with results related to patient and nurse satisfaction

Midwestern, 532-bed, acute care, tertiary, Magnet® designated teaching hospital;
67 nursing staff members and EHRs

Not applicable (not explicitly stated in the article)

The study was conducted at an unnamed academic Medical Center across 5 units (acute care, critical care, children’s hospital, women’s health, and nursing float pool); 791 nurses participated (out of 2705)

Methods: Intervention/Instruments

Literature reviews for English-language journals (2006-2016) in PubMed, CINAHL, and MEDLINE usig keywords: bedside nurse report, bedside handoff, bedside handover

The authors gathered fall rates, patient satisfaction, and nurse satisfaction data at the start of the research and implemented the program for four months and then gathered the same data. The data were analyzed statistically to determine if there are differences in the before and after data.

Literature review of published research on BSR and communication

The authors administered a 15-item survey through SurveyMonkey, where they sent the link to 2705 nurses in the Medical Center. The instrument used was based on the Assessment of Shift Report questionnaire, which included items that reflect the nurses’ perceptions of BSR.

Analysis

Patients’ experiences with bedside shift reports show that in general, patients have positive feedback on this type of handoff. However, the limitations and disadvantages include misinterpretations or misunderstanding of medical jargon, tiredness, information overload, privacy concerns, and inconsistencies. Nurses’ experiences, on the other hand, show that nurses who practice bedside shift reporting have positive experiences, but there are those who do not prefer it. This is because of fear of mistakes and lack of knowledge about how to facilitate these types of handoffs, which causes concern over patient welfare and job performance.

Numerical measures of patient safety and satisfaction, and nurse satisfaction were used to determine how effective bedside shift reporting is in acute care settings. SPSS was used to compute for independent samples t-test, Mann-Whitney U test, mmean computations, and Chi-square tests.

Communication during BSR has been established to be the key to improving BSR-related variables. The key outcomes related to BSR were examined in the context of how communication played a role in the improvement of the variables concerned.

Descriptive statistics using the Assessment of Shift Reports questionnaire, analyzed using Quantitative: Chi-square tests to determine significance between agreements/disagreements with statements from the questionnaire, and two-sample t-tests for analyzing patient perspectives and survey
Qualitative: Content analysis through coding and theming

Key Findings

Bedside shift reporting is time-consuming to implement, but has positive outcomes for nurses and patients

BSR was associated to better patient safety (decreased fall rates), improved patient satisfaction, and nurse satisfaction.

BSR has been found to be an effective communication tool between nurses and patients. BSR is slowly becoming a necessity as it improves knowledge and accountability.

Overall, the implementation of BSR yields positive responses from nurses in terms of its effectivity and efficiency as a means of communication, indicator for progreess and performance, accountability, and cohesiveness of information dissemination among nurses and transparency with patients. The main negative responses revolve around the stress about the process, still-existing delays in providing care, and the involvement of the patients.

Recommendations

The authors note that for a successful implementation of bedside shift reporting, nurses must have strong understanding of process management and good mentoring skills, as well as the affinity for relationship building.

The authors recommend that to prevent inconsistencies in BSR, nursing leaders need to maintain the process and continually educate and reeducate nurses for better compliance.

No recommendations were made by the authors.

The implementation of BSR in nursing almost always have gaps in different concerns. The on-hand experiences of nursing with its implementation shows that different units have different satisfaction ratings with regard to BSR, and it is recommended that applying EBP and nursing research be in tandem with implementing BSR guidelines.

Explanation of How the Article Supports EBP/Capstone Project

This article supports the rationale for my capstone project. The literature review conducted by the authors in their research show that there are benefits to implementing bedside shift reporting, and it also shows some possible points for improvement based on limitations or weaknesses of the process.

The article provides evidence and support on the measurable outcomes for implementing bedside shift reporting and proposing standard operating protocols for the process.

The article provides a solid support on how the communication during bedside shift reports improve patient safety, patient satisfaction, and nurse satisfaction.

This article provides insights on the personal experiences and inputs of acute care nurses regarding BSR. The personal experiences of nurses provides an evidence-based feedback system and helps in identifying the gaps in BSR implementation based on actual, practical events experienced first-hand by those involved in the process
