In the past 2 months, L.S. has experienced abdominal pain, diarrhea,a and fatigue. The patient’s mother reported that he had lost weight and appetite during that time.
Currently, he makes many visits to the restroom.
.In 2015, he had perianal abscess. His mother and brother have been diagnosed with Abdominal pain assessment were rated as 8/10 and reports it is cramping.
He has lost 15 lbs in three months. The stool exam showed loose and watery stool with trace amounts of blood. L.S. takes ibuprofen for abdominal pain MiraLAX for occasional constipation.

Diagnosis
Acute r/t pain prolonged diarrhea and anal fissure
Crohn’s disease
Crohn’s disease shows perianal abscess and fistula and there is a likelihood of right lower quadrant tenderness (Ball et al., 2018). There is intestinal obstruction as evidence by abdominal pain and the patient has lost 15 lbs (The Ohio State University, n.d.).
Ulcerative Colitis
Bloody, frequent and watery diarrhea, weight loss, fatigue are common in patients with Ulcerative Colitis (Ball et al., 2018).
