The management practices have multiple similarities and differences, although they are engineered to a common goal of enhancing the health care sector. The management practices are applicable in different scenarios depending on the health care provider and the level of treatment required. Disease management interventions are needed to manage and control epidemics and diseases effectively. Disease, case, and outcomes management have specific differences in their application, care practice, and management process. These management practices significantly influence the healthcare system, and their continued application in treatment is necessary. The paper focuses on the chief similarities and differences of Case, Disease, and Outcomes management practices in healthcare. Further, it emphasizes their success and relevance in healthcare today and in the future.
Similarities among Case, Disease, and Outcomes Management
Overall, reducing healthcare costs by assisting patients in taking charge of their health and avoiding overuse is made possible through case management, outcomes management, and disease management. All the management approaches are suitable in ensuring patients get affordable healthcare services. Affordable healthcare services are related to adequate care provision with reduced costs. Patients or healthcare providers are concerned about issues that may arise at the interfaces between the two types of care: outpatient and inpatient (Hudon et al., 2019). Patients with chronic conditions may be at risk due to a lack of evidence transfer and organizational issues. According to evidence-based guidelines, asthma management, for example, does not adequately meet the needs of those with the condition. Patients with diabetes can benefit from better integration of research findings into routine care. Patients with chronic conditions often require care from various professionals, so better coordination among healthcare providers is necessary to ensure continuity.
Another similarity entails their application such that in all management practices, organizations and providers must define what they seek to achieve and identify achievable improvement levels. Determining the level of care to offer the patients is vital for any management practice to be considered. Coordinated efforts are required to effectively manage patient cases while developing an excellent physician-patient relationship for adequate care provision (Hudon et al., 2019). Care plans require an assessment of the factors that influence applying these management practices in the nursing setting. A better understanding of how these three management practices work is essential in adopting relevant measures and interventions to enhance patient quality of care (Mantravadi, 2016).
All three management practices are associated with process and outcomes measurement. Concerning case management, the case managers engage in a collaborative process of assessing, planning, monitoring, and discharging care to patients. The practices and activities measure the process and outcomes obtained from every step of coordination. Disease management also focuses on attaining quality care and patient knowledge on dealing with different conditions (Powell & Tahan, 2018). This is achieved through a series of coordinated activities and processes for building the physician-patient relationship. Outcomes management itself is a process encompassing both disease and case management in its application and practice. The outcomes management practice comes from the other two management practices since it is a broad concept from which the other practices evolve. Outcome measurement is thus applicable in all management practices.
All the management practices are multidisciplinary and accommodate collaborative approaches. Care delivery employs a collaborative perspective in managing various conditions. Joint assessments and care planning, informed by the goals and decisions of service users, are carried out. Improvements in communication and information sharing across the team and with the service user increased participation of the service user, or their caregivers, in decision-making about their care single point of access through a key worker (Powell & Tahan, 2018). Rapid access to specialist expertise in the community, including urgent care in a crisis and transitions of care, and quick access to expert knowledge is enhanced. A variety of community services that promote well-being, consciousness, and disease prevention is available.
