Introduction
ABC provides therapeutic foster care to children with behavioral or mental health problems. This is a residential treatment designed to offer client-based therapeutic services for children with severe psychosocial challenges. The residential intervention is created to make certain that all care services are responsive to the needs of children under treatment. Children are efficiently helped when their guardians/families are actively participating in development and implementation of the care intervention plan. Effective treatment outcomes are accomplished when treatment plans and objectives are based on the individual needs and strengths of each child together with their family support (Brown, McCauley, Navalta, and Saxe, 2013). In this study, a therapeutic milieu practice is set up showing how the care environment was developed to address psychosocial conditions of a child.
Physical milieu
ABC can give accommodation to a maximum of 75 children living under care program. The residential services are offered to children from 7 to 15 years of age of both genders. Children admitted to this program are those who experience severe mental-health related challenges which interfere with their wellbeing in school, at home or in the community. Children referred to this residential intervention need more therapeutic services than can be offered in more traditional treatment settings. The residential services are only given to children with severe or complex nature of challenges. According to Glassman and Flaherty (2013), such challenges may include conditions such as persistent or severe diagnosable mental ailments, unique social, behavioral, emotional and psychological needs, crucial mental health challenges or diagnosed disorders, continuous conflict experienced in the home environment causing adoption challenges or high risk behaviors. The family must be willing to involve in all aspects of identified treatment intervention in order for a child to be accepted into the therapeutic program.
ABC residential services can be obtained at the county level. ABC may negotiate with other children’s mental health services within the county to offer residential services. Apart from such negotiated agreements, ABC does not admit direct, external referrals. Any kind of referral is authorized from care staffs of ABC and only after taking careful assessments to make certain that all other effective community-based interventions have been considered or tried. All treatment interventions focus on the strengths of the children, are culturally responsive, and encourage shared decision-making (Brown, McCauley, Navalta, and Saxe, 2013). The treatment is client-centered considering the entire context (individual, family and community) perspectives; therefore respects the preferences and needs of the children.
Cultural milieu
The mental health services that ABC provides to children respect the diversity that exists within every community living in the country. ABC strives to achieve inclusion to ensure that all children are treated equally and taking into consideration of sexual orientation, racial, linguistic, ethnocultural, and religion diversity. Accommodations for every child is recognized and put into documentation in an individual plan care (Glassman and Flaherty, 2013). Every effort is committed to accommodate all special needs. Actions are developed to support the identified needs by the use of the individual plan care. These plans elaborate the roles of each party and are evaluated monthly during the monthly review meetings.
Brown, McCauley, Navalta, and Saxe (2013) explain that arrangements are organized for the children to meet special needs like the need to access particular recreational activities which promote cultural, social, and physical benefits for every child in care. Children are advised to maintain positive involvement and interaction in their cultural community. Religious and racial discrimination are discouraged.
Ideological milieu
ABC utilizes the sanctuary model as a tool to develop a trauma-sensitive culture and for creating the protective factors essential for its staffs to carry out the mandate of the organization. Founded on the trauma-sensitive culture, ABC employs this culture to strive towards ensuring that all members feel morally, psychologically, physically, and socially safe; in which all members handle their emotions effectively; identify and address grief and loss; and remains committed to developing positive future.
