Lack of insight has been found to be an important predictor of adverse clinical outcomes, treatment non-compliance, and increased cognitive impairment (Aleman et al., 2006; Mohamed et al., 2009; Yen et al., 2005). I agree with LHJ's assessment. Illness insight is complex and cannot be treated as a single prerequisite for psychiatric rehabilitation. The 'insight paradox' reminds us that understanding can improve treatment adherence and outcomes, but its effects vary by person. Mental illness affects everyone differently, so psychiatric rehabilitation should be individualized (World Health Organization, 2022). A more flexible and person-centered approach that treats insight as a variable rather than a rigid requirement is pragmatic and aligned with psychiatric rehabilitation's holistic care and individualization principles.
