Over the years, the healthcare system in the United States has been fragmented. As a result, healthcare professionals have significantly worked on implementing programs focused on coordinating the care provided to patients with chronic conditions (Ameri, 2015). However, the programs vary broadly in style and structure, although their primary goal has been to improve the outcomes of diseases while still minimizing the aggregate healthcare cost. While establishing healthcare programs, healthcare practitioners have experienced challenges in overcoming the gaps in providing quality care to patients with chronic illnesses.
Healthcare practitioners have experienced significant patient support efforts. As a result, medical practitioners have been willing to advance the wellbeing of patients by developing approaches focused on minimizing healthcare expenditure and improving the quality of care provided to patients with chronic conditions. The study conducted by Foo et al. (2020) indicates that there exist various challenges associated with managing chronic illnesses among patients. Such problems include healthcare system fragmentation, poor communication among care providers, and lack of adequate guidelines. Also, the challenges include a lack of skilled medical practitioners able to handle complicated and varied illnesses, engage in effective communication, make decisions agreed upon by caregivers and patients, and provide healthcare centered on patients. Moreover, the gap also includes a lack of adequate consultation time to discuss and review patients’ conditions. It should be noted that consultations are essential because they enable healthcare providers to provide quality care to patients. Physicians derive their satisfaction from preventing frequent admissions to hospitals and coordinating care.
